Summary of Job Duties: ***This position is located in Little Rock, AR at our Rahling Road clinic ***

The Nurse Educator will work within the identified Family Medicine Clinic to promote screenings, early identification, and clinical navigation for the health and development of children prenatal through 5 years. Additionally, the Nurse Educator will be responsible for managing relationships with program participants, families and local community-based organizations. This person will track program progress and timeline deliverables to ensure the goals and objectives of the project are met, while also ensuring that relationships with participants and institutional stakeholders are maintained throughout the project. This position will engage in a range of activities such as outreach and participant recruitment, participant screening and assessment, health education, care coordination/service navigation, and community education and engagement.
Qualifications:

Minimum Qualifications:

  • Associate degree in Nursing with seven (7) years experience as an educator/instructor or RN or BSN in Nursing plus five (5) years experience as an educator/instructor or RN.
  • Valid AR drivers' license required with current automobile insurance and reliable car for moderate travel to LR occasionally and to surrounding communities (mileage reimbursed at the current state mileage reimbursement rate) if a state vehicle is not available. Occasional out-of-state travel.
  • A personal working cell phone is required.
  • Must be able to work evenings and weekends as needed.

Knowledge, Skills, and Abilities:

  • Detail-oriented and Organized.
  • Excellent interpersonal skills.
  • Strong problem-solving skills.
  • Ability to work independently and interdependently.
  • Experience with Microsoft Office is required.
  • Excellent oral and written communication skills.
  • Ability to work under deadline pressure.

Preferred Qualifications:

  • Bilingual in Spanish or Marshallese


Additional Information:

Key Responsibilities: Promotes Screenings, Early Identification, and Clinical Navigation for Children Prenatal-5 years of age  
  • Implements and promotes the benefits and value of the program of the program to maintain a Patience and Family Centered Care approach.  
  • Develops a clinical strategy for Well Child Checks 0-5 years of age pre-visit planning and coordinates transition of care for pediatric patients.  
  • Effectively collaborates with nursing staff at all levels and participates with the clinic's complex care team to ensure children who are high-priority beneficiaries receive optimal care. 
  • Ensures screenings, assessments, and treatment planning for pediatric clients are conducted according to chronological and developmental age. 
  • Coordinate referrals, support services, and/or treatment pathways for pediatric patients and their families.  
  • Support continuous quality improvement within clinics.  
  • Provide training and continuing education opportunities for clinic and community on early childhood healthy development.  
Develops and manages relationships with program participants and community stakeholders.  
  • Coordinates communication with community stakeholders.
  • Responsible for relationship building and management with external partners.
  • Frequently tracks and reports on program deliverables through effective utilization of program management software.
  • Identifies barriers and utilizes good problem-solving skills to address issues that arise in regard to participant relationships, ensuring all program objectives are met, and data collection processes and procedures are followed.  
Inform program and support clinic decisions as related to culturally adapting services to the community.   Other duties as assigned
