Summary of Job Duties: This position is responsible for providing performance oversight of Population Health and value-based care programs and services in the system's primary care clinics. Under the direction of the Director of Quality for Population Health and Value Based Care. The Population Health Project Manager interfaces with internal stakeholders and all payer program representatives to ensure all aspects of required programs are met, Maximizes Value Based Revenue, and achieve better population health for UAMS patients. The PHPM provides management of initiatives and projects related to Quality Care, Safety, and Patient experience including efforts to standardize/improve care, close patient gaps in care, and improve patient outreach and engagement.
Qualifications:

Qualifications:

  • Baccalaureate Degree, Major: Health Info Management, Health Education, Public Health, Statistics, or related field
  • Administrative experience: 5 years
  • Computer program experience: __Spreadsheets, Database, MS Suite, and Project Management
  • Quality Improvement Experience
  • Insurance/Payer Program Experience

  • Works closely with the Quality Team at each site to ensure program/project expectations are clear, concerns are addressed, and objectives are met.
  • Organizes and maintains regular project meetings including meeting minutes, agendas, and project management tracking and reporting documents.
  • Support UAMS Primary Care Clinics by managing the portfolio of Population Health and Value Based Care Programs, initiatives, and project for optimized performance. 
  • Establish and maintain a working knowledge of Improvement Science and all value-based arrangements and programs UAMS is currently participating and provide input, communication, and education on these programs and performance to multiple internal and external stakeholders.
  • Compile and maintains accurate records for all efforts related to Population Health and Value Based Care Quality, ensuring all documentation in payer portals is entered completed accurately and timely, the status of internal projects is clear and reportable, and any other needed data and documentation is neatly organized and available upon request.
  • Participate in Quality Improvement (QI) Committee by conducting QI Audits and reporting results. Will attend meetings at UAMS related to quality improvement as needed.   
  • Utilize the Epic and Power BI reports, along with payer related portals and reports to identify trends, outliers, and track progress.
  • Establish and maintain a working knowledge of all value-based arrangements and programs UAMS is currently participating in and provide input, communication, and education on these programs and performance to multiple internal and external stakeholders
