Position Summary & Responsibilities:



The Registered Nurse Inpatient Rehab Case Manager, under general supervision of the Program Director of Inpatient Rehabilitation, is responsible for leading care coordination and assessment of discharge plan for patients requiring inpatient rehabilitation. Concurrently screens patients to identify and facilitate discharge planning needs.  Coordinates discharge plans with individual patients, families, physicians and other members of the health team.  Functions in a case management role for the Inpatient Rehab Unit, coordinating the care and service of the rehab population across the continuum, promote effective utilization, monitor health care resources, and assume a leadership role with the interdisciplinary team to achieve optimal outcomes using the tools of case part of a collaborative process that assesses plans, implements, coordinates, monitors and evaluations health care delivery systems.   Based on the needs and values of patients and in collaboration with all service providers, the case manager links patients with appropriate resources throughout the continuum of health care settings to assist

Minimum Position Requirements:

  1. Licensure/Certification/Registration:  
    1. Current Texas RN License or Compact State RN license
    2. Advanced certification preferred (IE Certified Rehab RN or Certified Case Management)


  1. Education:  
    1. Associated degree required; BSN preferred; must enroll in a BSN program within 6 months of hire and obtain within 36 month


  1. Experience:
    1. Clinical experience in the care and management of the Inpatient Rehabilitation patient.
    2. Minimum of  one year previous RN experience within an acute medical or rehab healthcare setting preferred
