The Wounded Warrior Project (WWP) Complex Case Coordination (C3) Program Discharge Planning Specialist oversees the admission needs and development of post discharge plans in collaboration with private and Veteran Administration facilities, C3 team managers and liaisons, and the Independence Program team. The Discharge Planning Specialist provides care coordination and long-term case management for high acuity cases and responds to referrals of warriors in crisis, conducts initial assessments, and refers Warriors to appropriate WWP internal resources or coordinates with external resources, as appropriate. The C3 Discharge Planning Specialist serves as a liaison and advocate to ensure rapid, coordinated, and collaborative response to warriors with difficult, complex, urgent issues that exceed the current capability or scope of any specific WWP program and coordinates and advocates with local resources, Federal resources (i.e., Veterans Administration, Dept of Defense, etc.), Veteran Service Organizations, and private facilities to establish an effective and comprehensive plan for the Warrior.


 

DUTIES & RESPONSIBILITIES

Caseload management:

  • Provide professional coordination of care for high acuity and long-term cases in need of extensive or specialized treatment.

  • Maintain a caseload of assigned warriors.

  • Perform comprehensive screenings and conduct needs assessments to identify and coordinate appropriate external care for warriors dealing with complex mental and physical health issues requiring immediate intervention.

  • Partner closely with warriors' care teams to develop and implement treatment plans aligned with the warrior's goals.

  • Collect and maintain sensitive protected health information according to organizational standards as required to coordinate warrior care.

  • Coordinate proper paperwork for warrior's screening, admission to approved care facilities, and connection with travel resources.

  • Ensure continuity of care by identifying and coordinating the necessary internal and external resources for warriors facing urgent and complex issues that require immediate intervention.

  • Collaborate with warriors, their household, and C3 liaisons to verify all parties understand the full spectrum of case progression from admission plan to post discharge planning to ensure concerns are addressed in a compassionate and effective manner.

  • Oversee coordination of appropriate step-down treatment and resources to ensure warriors receive the assistance needed for their long-term support as they continue their journey to recovery.

  • Provide planning, coordination, execution, and participation in mental health programs to enhance the quality of warriors' resiliency and psychological well-being.

  • Develop and encourage a collaborative relationship with warriors to maximize the warrior's ability to make informed decisions, utilizing a warrior-centric model, within organizational parameters and serve as the WWP point of contact, following up with warriors throughout their care, treatment, and recovery.

  • Ensure case documentation and data are up to date and accurate. Review warrior referrals with C3 Liaisons in support of discharge plans.

Program management and development:

  • Establish ongoing care and caregiver support and advisement to liaisons and managers.

  • Refer warrior and/or warrior's family to internal resources (WWP programs) as well as external resources (i.e., VA, etc.) to meet the needs of warriors and facilitate the recovery process.

  • Facilitate comprehensive support and care for warriors by orchestrating involvement in one or multiple internal WWP programs or external organizations, such as the Veteran's Administration (VA), Community Care Network Providers, and social services agencies.

  • Empower warriors for lasting recovery by mitigating the challenges that precipitated the crisis while removing barriers to care to enable the warrior to receive needed assistance.

  • Collect necessary documentation to facilitate payment for WWP-funded care and manage additional invoices needed to request payments on behalf of warriors and their households, in accordance with WWP guidelines.

  • Work with Department of Defense (DoD), Veterans Benefits Administration (VBA) and Veterans Health Administration (VHA) to coordinate professional services and care for C3 Warriors.

  • Complete intakes to determine eligibility into contracted care facilities, DOD programming, VA Polytrauma and provide recommendations regarding program enrollment.

  • Prepare briefings and reports for stakeholders.

  • Provide feedback to Program leadership on process improvement and system needs.

Resource management and development:

  • Serve as a subject-matter expert specializing in medically complex cases, including but not limited to traumatic brain injuries (TBI), and high-risk individuals.

  • Cultivate strong and productive partnerships with internal programs, external governmental agencies, private facilities, community resources, and Veteran Service Organizations to ensure effective collaboration and support for program initiatives.

  • Research programs, services, resources, recreation, transportation, education, day programs, etc. to provide appropriate resources to warriors served in the Complex Case Coordination Program. Conduct research on diverse programs, services, resources, recreational opportunities, transportation options, educational facilities, and day programs. These efforts aim to deliver tailored and fitting resources to warriors in the C3 Program, ensuring their specific needs are met comprehensively.

  • Assist with communication coordination among family members, caregivers, WWP program staff, other external organizations, agencies, and healthcare facilities.

  • Other related duties as assigned.

KNOWLEDGE, SKILLS, & ABILITIES

  • Demonstrated understanding of post-discharge care requirements.

  • Strong analytical skills. Demonstrated ability to identify problems, analyze information, evaluate results, and solve problems using the most effective solutions.

  • Demonstrated understanding of local community and government mental health resources.

  • Knowledge of developments in research, treatments, policies, and resources that impact C3 warriors and their families.

  • Demonstrated understanding of military and veteran issues, e.g., brain injury, post-traumatic stress disorder (PTSD), physical and mental health.

  • Demonstrated understanding of mental health conditions, treatment modalities, facility programs, and resources available to the Warrior.

  • Comprehensive understanding of Veterans Administration Polytrauma care and resources.

  • Demonstrated understanding of the social perspective of warriors.

  • Ability to communicate with diverse personalities in a tactful, mature, and flexible manner, handling highly sensitive matters with diplomacy, empathy, and confidentiality.

  • Demonstrated ability to develop and maintain relationships with internal programs and external partners.

  • Demonstrated understanding of Wounded Warrior Project and its mission, values, programs, and services, and the impact of these programs and services on wounded warriors and their families.

  • Strong written and verbal communication skills.

  • Strong organizational and time management skills

  • Proficiency with Microsoft Office Suite, Microsoft Teams, and Salesforce.

  • High energy level. Self-starter; willing to take initiative.

EXPERIENCE

Requirements

  • Four years of experience in roles such as nurse case manager, social worker, post-discharge planning involving case management, or other related fields.

  • Four years of experience working with veteran healthcare-focused programs and case management.

  • Four years of experience working with military and veterans' population in support of their development and readjustment.

  • Proficient in Salesforce or other CRM system.

Preferences

  • Five years of experience working with local community and government resources providing services for warriors in crisis.

  • Five years of experience working with and providing support to wounded service members who are dealing with both visible and invisible injuries, e.g., brain injury, post-traumatic stress disorder, and combat stress.

  • Five years of experience as a nurse case manager or social worker.

EDUCATION

Requirements

  • Master's degree in nursing, social work, psychology, rehabilitation counseling, case management, health services administration, or related mental health field.​​ Equivalent combination of applicable education, training, certification, and experience working with veteran focused programming or case management may be considered in lieu of degree.

CERTIFICATIONS & LICENSURE

Requirements

  • Valid state-issued driver's license.

  • Ability to gain the Certified Brain Injury Specialist (CBIS) credential within 2 years of hire.

WORK ENVIRONMENT/PHYSICAL DEMANDS
