We are committed to making a difference for our neighbors, friends, and family and our vision is to be a trusted, nationally-recognized leader of innovative, collaborative community health.    Employment Status: Salary Exempt Working Hours: 40 hours a week (80 hours per pay period) Shift: 1st shift Position Reports to: Home Health/Hospice Director                            Department Description: Wilson Health's Home Health & Hospice Care is designed to provide comprehensive healthcare to patients in their own home on a temporary basis. The care is provided under the direction of a physician, and the goal is to help patients regain their independence after an illness, injury or hospitalization. The goal for patients with chronic illnesses may be to keep them at home safely and as pain-free as possible. Services provided are Skilled Nursing Care, Home Health Aide Care, Social Worker Care, Physical Therapy Care, Occupational Therapy Care, and Speech Therapy Care.   Job Summary: Ensures quality and safe delivery of home care and hospice services.  Develops and implements effective work processes. Promotes and maintains an agency environment compliant with federal, state, and local agencies. Develops, implements, and evaluates quality management program. Participates in professional growth and development. Promotes effective written/verbal communication. Other duties may be assigned.    Essential Duties & Responsibilities:
  • Ensures quality and safe delivery of home care and hospice services.
  • Develops, implements, coordinates, and evaluates the quality management program.
  • Compiles and disseminates monthly, quarterly, and annual reports encompassing safety, infection, customer satisfaction, and quality.
  • Chairs the quality management committee for home care and hospice
  • Tracks, analyzes, quantifies, and compares data relative to safety, infection, customer satisfaction, and quality.
  • Acts as a liaison between the organization and third party payer.
  • Conducts chart audits for ongoing performance evaluations.
  • Oversees the surveillance for infections of staff, clients, and caregivers. 
  • Facilitates staff education designed to improve care delivery processes and client outcomes.
  • Designated compliance officer.
  • Insures compliance for billing and regulatory requirements of Federal and State programs.
  • Responsible for OASIS program implementation, ongoing revisions, and staff OASIS competency.
  • Coordinates implementation & education of nursing staff for EMR (Electronic Medical Record) System
  • Performs 30 and 60 day and annual evaluations for staff.
  • Counsels staff as necessary.
  • Supervises field staff in home.
  • Monitors absence and tardiness of staff.
  • Monitors client complaints and counsels staff as needed
  • Assesses on-going quality education needs of staff members and conducts education programs as appropriate.
  • Assumes responsibility in absence of Clinical Supervisor(s).
  • Primarily will work M-F, but will work weekends/after hours sparingly based on need
Qualifications:
  • Graduate of an accredited school of nursing program (Must be Ohio licensed or have multi-state)
  • BSN required, but preference will be given to those with a Master's degree
  • Must have 5 years of clinical nursing experience in a hospital environment or Home Health/Hospice.  
  • Preference will be given to candidates with Home Health/Hospice experience
  • Candidates with previous supervisory/management preferred
  • Candidates with previous quality experience, education, and billing also a plus.
