Description
This is full time position working Monday-Friday, with a Sunday on call rotation. reports to the Director of Case Management. This social worker provides direct social work intervention, transition planning and counseling to patients and their families. This applicant will assess the patients social, emotional, financial, cultural, and situation. The social worker will interpret this information to the physician, nursing staff, and other care team members to facilitate appropriate transition planning. The social worker will assist patients and families in making appropriate linkages to community resources and provide consultation to those seeking input and guidance in efforts to resolve patient concerns. The applicant will be cross trained to other areas of the facility.
Qualifications:
- Currently licensed to practice as a social worker within the State of Ohio.
- Bachelor's degree required masters preferred. LSW required, LISW preferred
- Social Services & Discharge Planning experience preferred.
Skill and Ability:
- Interpersonal and analytical skills necessary to function under the current standards of Social Services.
- Computer skills that match the needs of the position.
- Physically able to perform the functions of the position.
- Maintains all required qualifications for area assigned.
- Ability to make psychosocial assessments and develop and implement viable care plans
- Excellent listening and communication skills
- All other duties as assigned.
