Be a part of a world-class academic healthcare system at UChicago Medicine as an Assistant Director for Ambulatory Social Care Integration at our main medical campus.

 

The Assistant Director for Ambulatory Social Care Integration will provide management and supervision to several clinical teams that support social care integration across the continuum of care between inpatient, ambulatory practice, and community services. The Assistant Director will oversee a team of ambulatory social workers deployed in several practice sites in the UCMC campus. The AD will also oversee a team of social workers that function as integrative Behavioral Health Care Managers that support optimal depression and anxiety management in primary care and selected specialty practices. The Assistant Director will oversee a team of community health workers and patient navigation coordinators that support patients to coordinate care with primary care providers on discharge from the hospital and in the ambulatory care setting to avoid hospitalizations, ED visits and readmissions. The Assistant Director will assist the Director in the day to day functioning of the department through scheduling, quality assurance, problem solving, and recruitment, evaluation of staff competency and performance and maintenance of the relevant department budget. The Assistant Director will be responsible for creating and revising standard work for each of the above clinical teams and implementing operational workflows to optimize departmental key performance indicators. The Assistant Director will establish relationships with other health care professionals and community groups and coordinate the services of the department relating to other relevant agencies. The Assistant Director is responsible for ensuring all team members demonstrate respect/appreciation for persons served by the Department, regardless of background, race, age, sexual orientation, gender, religion or disability; treats all associates fairly and equitably; remains open to divergent opinions; and seeks a culturally, racially and religiously diverse department. Promotes and has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial and developmental needs of neonatal, pediatric, adult and geriatric patients.  Capable of communicating with individuals in age specific terms and language.

 

Essential Job Duties:

  • Manages the clinical practice of Social Workers, Community Health Workers, Patient Navigation Coordinators in social and clinical care delivery  in the UCMC adult ambulatory clinics and discharging from UCMC Hospital, ensuring quality of service in care integration among UCMC hospital and providers/community providers/Community service organizations. Provides clinical supervision and oversight of staff Social Workers, Behavioral Health Case Managers, Community Health Workers, and Patient Navigation Coordinators and takes primary responsibility for the training and orientation and on-boarding of new staff to the Transformative Care Department.
  • Establishes and implements a KPI dashboard for each program under management and sets annual targets for performance with the Director of Ambulatory Care Coordination.
  • Ensures that supportive care coordination services are delivered to patients in a timely manner by developing well-trained, dedicated staff who are available during normal business hours.
  • Responsible for collaborating with and assisting the Inpatient Care Management, UR, finance teams, and ambulatory clinics n connecting patients who are out of network with appropriate medical homes, collaborating with care teams on Length of Stay and Readmission reduction, and Denials management task forces to optimize patient care delivery.
  • Available to support case management, social work, community health worker, and patient navigation coordinator staff. Is an active, experienced problem-solver. Supports team to complete compliant medical chart documentation as required.

  • Maintains awareness and knowledge of current clinical issues, regulatory processes, or advances relevant to departmental decision-making and functioning.  Participates in regular continuing education.  As directed by leadership, participates in regional and national professional meetings, representing the department. Understands changes in health care delivery and payor systems within health care systems.
  • Supports the Director in managing the department within the frame work of the annual budget.

QUALIFICATIONS:

Licensed Clinical Social Worker required

 

Strong clinical background, with minimally 5 years of experience in Care Management or Ambulatory Social Work

Demonstrate knowledge and experience in project management, effective managerial skills and principles surrounding quality improvement and utilization management. 

Strong computer skills (MS Office, Word, Excel) and experience with data management necessary.  A “hands on”, visual, team-builder and effective communicator style is key to success of position.

Current working knowledge of case management and public health necessary.

 

LICENSES/CERTIFICATIONS:
  •   It is preferred that Case Management Certification is obtained within two years of hire.
