Summary of Job Duties: The Registered Nurse will provide direct patient care as an RN. This professional nursing role will require the ability to coordinate care for patients, triage patients, and work collaboratively with clinic providers, fellows, and staff. Requires the ability to effectively work with patient referral sources, facilitate appointments, coordinate transitions of care, and provide patient education on complex conditions. The RN will work with the individual patients, their families, and providers of care to develop and implement individual health plans.
Qualifications:

Minimum Qualifications:

  • Education, general: Bachelor's degree in Nursing, Associate's degree in Nursing, Diploma RN, or Master's degree in Nursing.
  • Education, special or professional: Must be a graduate of an accredited RN nursing program.
  • Experience, amount (in years) and type: Minimum of 1 year of Registered Nursing experience.
  • Licenses: Valid Registered Nursing License
  • Special knowledge, abilities and skills: Basic computer skills. Excellent communication skills. Strong work ethics and accountability.
  • Physical Requirements: Employees in this role must be fitted for, capable of donning and performing routine tasks in personal protective equipment to include N95 masks, Powered Air Purifying Respirators (PAPR) or Controlled Air Purifying Respirators (CAPR).

Preferred Qualifications:

  • Education/Experience preference: Education, experience, classes, or degree focusing on cardiac devices such as pacemakers, ICDs, and ILRs highly preferred.
  • Licenses, certificates, or registration: • Current BLS • ACLS certification (or obtain certification within one year of starting in this role)

Additional Information:

  • The RN supports patient care in the cardiac device and remote monitoring environment for both outpatient and inpatient settings for patients with cardiac electronic devices, pacemakers, cardiac defibrillators, loop recorders, and holter monitors for diagnosing and treating cardiac arrhythmias.  
  • The position will collaborate with Cardiologists, Electrophysiologist, Advanced Practitioners, cardiac device technologists, and other members of the patient care team to achieve optimal clinical outcomes.  
  • Uses an integrated approach toward patient outcomes. 
  • The Registered Nurse will promote wellness; improve outcomes, and self-care within a defined population of patients.  
  • The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state.
  • Will work with the individual patients, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management.  
  • Coordinate care and/or follow-up for patients who receive care in outpatient facilities.  
  • Coordination of prescription refill requests and patient education coordination in collaboration with physician staff.
  • Commitment to patient outcomes results in high levels of performance despite workload changes.
