Summary of Job Duties: The Care Management Social Worker Lead is responsible for assisting the Social Work Manager.
The Care Management Social Worker Lead is a member of the care management team and responsible for assisting the Social Work Manager and team in the management of the continuum of care in the inpatient setting. The role reflects appropriate knowledge of Social Work scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, and other Federal or State regulatory agency requirements specific to the Social Work role. The Social Work team member will partner with the multi-disciplinary team utilizing best practices, relevant data and compliance with the mission/philosophy, standards, goals, and core values of UAMS. Primary elements of the social work role include assessment of the psychological, financial, and discharge planning needs of the patients and families, providing information, support, counseling, and referrals to other levels of care and appropriate resources for a variety of age-specific populations. The Social Worker Lead will work in collaboration with Care Management Leadership and other organizational leaders to ensure regulatory compliance and provide oversight of resource consumption appropriateness.

This position offers a $10,000 taxable Sign-On Bonus in return for a 18-month work commitment
• $3,000 after completion of 90 day probation
• $3,000 after completion of 1st year of employment
• $4,000 after completion of 18 months of employment

Qualifications:
• Applicant has applied for a full-time Social Work position on an Inpatient Unit or in PRI in the Care Management Department
• Applicant is not a current UAMS employee
• If applicant was previously employed by UAMS, at least 180 days have passed between termination and re-hire

Payment Eligibility Requirements for Applicant:
• Must maintain full-time employment at UAMS.
• Must remain employed in Care Management Department.
• Must maintain satisfactory annual performance evaluation with no progressive discipline. Must be hired and remain employed at UAMS to receive payments
Qualifications:

Minimum:

  • Master's Degree in Social Work (LMSW)

  • Must be licensed by an Arkansas Professional Board as a Licensed Master Social Worker (LMSW)

  • 1 year of Social Work Case Management Experience

Desired or Preferred:

  • Care Management Certification

  • Licensed Certified Social Worker (LCSW)

RESPONSIBILITIES:

  • Performs the duties of the Social Worker Care Manager

  • Assures hospital Care Management activities are effective in maximizing patient satisfaction, utilization of resources and third-party payer reimbursement

  • Identifies, intervenes on taking a proactive approach to ensure right time discharges and documents avoidable and denied days and discharge delays.

  • Identifies patient/families with psychosocial and/or non-medical discharge planning needs and refers to the Social Work team member as appropriate.

  • Provides team coverage during times of staffing shortages (sick/vacation/etc.)

  • To lead evidenced-based initiatives and strategies, that support the appropriate acceptance, patient placement and use of clinical resources

  • Participates in Interdisciplinary Care Rounds (IDCR) to ensure each patient receives the correct resources at the correct level of care at the correct time in their acute care episode

  • Participates in the performance evaluations of the team members including goal identification, coaching, action plans and counseling/disciplinary process as required.

  • Maintain high reliability culture among team/ point of contact for team (being prepared for ADH, Joint Commission, etc.)

  • Serves as a liaison to other departments to promote coordination of care for patients.

  • Responsible for collecting weekly Complex Case Review information, organizing schedule for presentations and ensuring documentation is completed.

  • Value analysis liaison considering organizational implications and logistics.

  • Oversees quality monitoring/data and provides guidance/suggestions for improvement.

  • Provides education to staff related to changes in policy/procedures, processes, EMR documentation, etc.

  • Manages orientation processes for new employees and provides feedback to managers for evaluations.

  • Assists with scheduling and daily staffing assignments; modifies assignments as needed.

Other duties may be assigned in order to meet the UAMS core values and business needs of the division.

Education/Quality Improvement Activities - Participates in Performance Improvement, Evidenced Based Practice/Research process. Supports the integration of current evidence into nursing practices. Promotes an environment conducive to positive educational experience for all students and staff. Provides effective patient/family education (link to PFCC).

Communication/Collaboration - Implements POC using an approach that is interdisciplinary with Patient and Family Centered Care. Effectively collaborates and communicates with the multidisciplinary team at all levels. Communicates care appropriately through documentation in the medical record. Reports variances in care/systems/processes through the appropriate means.

Customer Service:  Interacts with and assists the public in a professional and friendly manner as needed. Demonstrates effective communication skills; communicates accurate and complete information; maintains strict confidentiality when necessary. Demonstrates positive working relationships with co-workers, management team, and ancillary departments; follows the Core Concepts of Patient and Family Centered Care and the Basic Code of Conduct Guidelines. Assist internal and external stakeholders as needed with exceptional customer service.
