The Continuing Care Center at Thompson is looking for a motivated, energetic individual to join our team as the MDS/Quality Coordinator.

MAIN FUNCTION

  • Lives the CARES values at all times. Commitment, Action, Respect, Excellence, Service.
  • Responsible for pre-admission screening, determination of skilled coverage and overseeing accurate Minimum Data Set (MDS) coding and development of the Interdisciplinary Plan of Care.
  • Additionally this individual would be responsible for the coordination of the Medicare determination and certification requirements.
  • Works closely with the Director of Clinical Compliance and the VP of Nursing for Senior Living Services to support the mission and strategic imperatives of Thompson Health.
REQUIRED JOB SPECIFIC COMPETENCIES

  • Pre-admission screening of patients/residents to be admitted to the Post Hospital Avenue and others as requested
  • Makes determination of skilled services upon admission and ensures Medicare/Medicare HMO certification is completed with the providers. Communicates determination to the admissions office.
  • Strategically sets assessment reference dates (ARD) in conjunction with the interdisciplinary team to capture needs, care and services delivered to most accurately reflect the status of the resident. Communicates ARD to Clinical Reimbursement Specialist for scheduling and initiation.
  • Serves as a resource to the interdisciplinary team for questions regarding MDS completion
  • Educates and trains new staff in the RAI process and MDS completion in the software program.
  • Performs in service training for staff regarding aspects of documentation that support the MDS completion per CMS regulations and RAI manual specifications.
  • Assures compliance with federal, state, Medicare, Medicaid regulations and MM Ewing Continuing Care center standards regarding completion, accuracy and timely electronic submission.
  • Accurately completes nursing portions of the MDS' on Post Hospital patients. Ensures timely completion.
  • Attends biweekly Rehab meetings and monitors compliance with documentation.
  • Reviews daily report for opportunities for case mix capture during Medicaid reimbursement window.
  • Maintains awareness of the MDS status of each patient/resident according to the Omnibus Budget Reconciliation Act (OBRA), Medicare – Prospective payment system (PPS) and Medicaid payment system.
QUALIFICATIONS  
  • Required - Current nursing license
  • Preferred – RN, BSN
  • LPN with 5+ years MDS experience would be considered
EDUCATION  
  • Preferred - MDS certification/ Medicare proficient
  EXPERIENCE  
  • Required - 5 or > of direct nursing experience/2 or > years of long term care experience/ 1 or > management experience
  • Preferred – MDS / Medicare management / Case Mix experience / LTC experience
COMPLEXITY OF DUTIES  
  • Demonstrates skills in time management and independent decision making without supervision.
  • Experience working in cross-functional teams desired.
  • Excellent written and verbal communication skills required.
  • Demonstrates skills in education and training.
  • Demonstrates skills required to develop and meet established schedules and deadlines.

Pay Range: $26.50 - $40.00 (dependent on if RN or LPN candidate)

Starting Pay: Based on Experience
