Main Functions:

  • Works collaboratively with a team of Clinical Dietitians to provide comprehensive nutrition care in the long-term care setting.
  • Develops and implements individualized nutritional care plans for residents.
  • Documents all aspects of nutrition care using standardized documentation.
  • Demonstrates the CARES values at all times.

Responsibilities:

  • Demonstrates the ability to work independently and exhibits flexibility in decision making and prioritizing
  • Develops and implements individual care plans for long-term care residents
  • Provides on-going nutritional assessments, monitoring, evaluating and education to residents and families
  • Integrates evidenced based research in the education, practice and management of residents
  • Responsible for timely and pertinent documentation of long-term care avenues as directed by department policies and procedures, professional judgment, provider orders and staff recommendations
  • Demonstrates accountability for the proper use of residents' protected health information and adherence to institutional safety practices
  • Actively guards the confidentiality of sensitive information including but not limited to that of residents, staff, and the health system
  • Compliant with all regulatory requirements included but not limited to requirements by JCAHO, DOH, HIPPA, and OSHA
  • Maintains expertise by pursing pertinent continuing education opportunities
  • Addresses questions and concerns of residents and families and health professionals in accordance with evidence-based nutrition practice and critical thinking and assessment skills
  • Effectively communicates nutrition plans and rationale to team members both verbally and in the medical record
  • Participation in avenue specific, disease specific interdisciplinary rounds
  • Serves as a liaison between residents, avenues and food service staff

Qualifications:

    • Bachelor's degree required in accredited nutrition & dietetics or related field
    • Completion of Dietetic Internship or ASCEND approved Graduate Program
    • Registery eligibility required
    • Registered Dietitian and CD-N preferred
    • One year in health care setting as a Dietitian or Diet Tech preferred

 

Pay Range: $28.00 - $35.00

Starting Pay: Based on Experience
