Home Care Scheduler - Onsite Pewaukee - .9 FTE

Schedule Details: This .9 FTE commits to 36 hours per week, 4 full days and 1 half day per week. Holiday rotation required, though only work half day each holiday. This position is required to work onsite at the Riverwood Admin building. This is not a seasonal or temporary position.

Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.

Coordinates staff schedules with PTO, holiday and weekend rotation. Education High School Diploma - or equivalent What You Will Do:
The Home Health Scheduler is responsible for coordinating all activities related to scheduling staff and patient assignments for ProHealth Home Care and Hospice. Specifically, accurate scheduling, registering and entering data of patient information into the Epic system.
What you will need:
- High school diploma or equivalent.
- At least 1 year of related scheduling and/or health care experience preferred.
- Experience with Epic EMR preferred.
