Job Summary

Provides direct patient care, education and support to patients in the medical day treatment setting. Functions as a clinical and community resource, educator and consultant. Able to coordinate and provide care for patients based on detailed treatment protocols initiated by physicians without need of direct physician supervision.

Accountabilities

  • Provides direct patient care in a medical day treatment setting. Maintains an advanced knowledge base of disease entities, medications and other treatment options specific to the medical diagnosis. Assesses and evaluates extensive health history. Evaluates effectiveness of care and makes changes according to predetermined care plan or after consultation with physician or appropriate medical/clinical provider.
  • Serves as primary contact for patient/family members in either a primary nurse role or  as a care giver for the day. Uses critical thinking skills and advanced knowledge to both deliver routine care  and to handle unexpected  issues. Contacts family and/or medical providers when the  need arises .
  • Participates in the intake of new patients either in the  role of a primary nurse or as a daily caregiver.  Takes part in admission meetings, assures that patient orders and required paperwork are complete, reviews new patient charts and communications when caring for them.  Works with patients and families  to help acclimate them to unit routines in a supportive manner.  
  • Identifies patient /family needs, and develops plan of care that includes medical, emotional, therapeutic, spiritual, educational and growth and development principles.  Is aware of established patient/family plan of care for assigned patients.  Plans daily patient care and activities according to care plan, and  documents  interventions, care and activities in the daily flowsheet.  Provides identified family education and documents interactions.   Updates (or communicates to primary nurse) and reviews patient care plans as needed.
  • Performs and/or assists with medical procedures, interventions, and therapies as prescribed. Provides comfort and safety measures for patient before, during, and after interventions.
  • Assumes responsibility for patient safety by maintaining a safe environment to include awareness of safety procedures, protocols, and adherence to patient care standards. Participates in environmental rounds to ensure safe environment. Utilizes both unit and individualized patient equipment according  to recommendations.  Utilizes the Patient Safety Net online reporting tool as circumstances dictate.    
  • Champions the development, implementation and evaluation of PI projects and/or new unit tools designed for the specialty area. Assists new team members until they are proficient.  Provides productive feedback and input when new tools are introduced to the unit.  Becomes a role model for the sustainability  of new ideas, processes and tools until  they become embedded in culture of unit.
  • Contributes to upkeep of unit by completing required daily and monthly checklists and duties.  Is knowledgeable about regulatory agencies and contract  pertaining to the Wonder Center  regulation and adheres to required standards.   
  • Acts as a resource for and coordinates continuing education for team members.  Presents formal  or informal  education to co-workers in the form of an approved DSS Daycare training program, a planned in-service, or as an informal resource on a relevant topic.

Supervisory/Management Responsibilities

This is a non-management job that report to a supervisor, manager, director or executive.

Minimum Requirements

Education - Associate's Degree in Nursing

Experience - 1 year of clinical nursing experience within pediatrics

Required Certifications/Registrations/Licenses

South Carolina Licensed RN - REGISTERED NURSE (Lawson Code NLRN

PEARS Certification or plan to complete in first 6 months of hire.

Certification in CPR/BLS

In Lieu Of The Minimum Requirements Listed Above

Team members employed in this job prior to July 1, 2020 are grandfathered under prior educational and experience requirements. In addition, team members must be certification eligible and will have one year to obtain required certification.
