Schedule is Monday-Friday with rotating shifts 6a-2p, 7a-3p. Some weekends required.
JOB SUMMARY
- Create the daily Hospitalist census to provide patient assignments for providers and hospital staff.
- Ensure patients have a primary care provider. Primary liaison between Hospitalists providers and primary care office.
- Assist with post-acute need resolution.
Qualifications
EDUCATION REQUIREMENTS
- 2 year / Associate Degree in Nursing from a state accredited school (Required)
- 4 year / Bachelor's Degree from a state accredited School of Professional Nursing. (Preferred)
- 2 - 3 years RN experience in acute care setting (Required)
- 2 - 3 years collecting information from diverse sources, applying professional principles in performing various analyses and summarizing the information and data in order to solve problems. (Required)
- 2 - 3 years resolving complex problems requiring use of basic scientific, mathematical, or technical principles and in-depth, experience-based knowledge. (Required)
- 2 - 3 years counseling and educating patients and their families, assigning and following-up on the work of other employees, and/or performing other duties requiring a comparable level of communication skill. (Required)
- 2 - 3 years comprehensive computer and database skills to streamline Care Management and communicate to other team members (Preferred)
- Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license, Cardiopulmonary Resuscitation (CPR)
