After successful completion of the probationary period, employee will receive off probation wage increase.


Summary:

This position is responsible for case management, coordination, and planning of appropriate internal and external care, program direction. (i.e. counseling, or therapy for child, adolescent, high risk, or dual diagnosed patients, etc.) .


Responsibilities:

* Provide comprehensive case management services with a focus on Oneida's child, adolescent, and chronic mentally ill patients.
* Effectively interview, screen, and assess clients' behavioral health needs utilizing treatment planning and case review methods.
* Make appropriate referrals and assist in the process of applying for services when necessary.
* Provide services to and be a liaison on behalf of clients who are cognitively, emotionally, or behaviorally impaired.
* Make home visits and serve as mediator between youth/adolescents, family members and parents.
* Assist in developing youth/adolescent recreational, social, and culturally specific activities. (i.e. leadership skills empower youth/adolescent support groups).
* Provide services consistent with HIPAA regulations.
* Provide transportation as deemed appropriate
* Be a positive role model for Oneida Community members who have little knowledge of the Behavioral health system.
* Ensure implementation of more efficient comprehensive, community integrated system of care and cost-effective treatment approaches to all clients.
* Develop a solid rapport with third party insurance providers for clear coverage justification.
* Prioritize multiple tasks and coordinate cohesive, unified case management within the Youth/Adolescents Resource and multi-disciplinary team.
* Provide outreach services and referrals to appropriate Tribal and County Agencies when child abuse or neglect is suspected.
* Attend pertinent County hearings related to your caseload.
* Implement and facilitate prevention services in conjunction with various Tribal and County prevention programs when appropriate
* Network with existing Tribal, County, and Private Sector Services that will best serve youths/adolescents and their family.
* Encourage and facilitate youth's participation in community and cultural activities (projects and services).
* Participate in department staff meetings.
* Maintain a current treatment plan for every active case assigned.
* Develop and maintain close, cooperative, and beneficial working relationships with students, parents, teachers, administrators, co-workers, peers, social workers, liaison officers, and other constituencies.
* Utilize school and community resources when needed to address the needs of student activities.
* Utilize development and training opportunities which will enhance services to all interested individuals.
* Meet with students to promote individual and group development opportunities.
* Consult with Oneida Cultural Heritage department staff on traditional methods of care.
* Must provide a series of regularly scheduled workshops, demonstrations on culture to youths and adolescents.
* Practice excellent customer service skills at all times to include, but not limited to addressing customer and employee needs courteously and promptly (Customer service provided shall be in line with the organizations "Good Mind" core values)
* Work remotely as needed per the Telecommuting SOP.
* Be proficient in your skill set in providing Telehealth Services.
* Be proficient in your skill set working within an Electronic Medical Record.
* Be proficient in grant writing and management as needed.

Skills:

Essential:
* Obtain additional education and training
* Comply with Caregiver Law and Grant conditions criminal history checks as a condition of employment to include fingerprinting
* Ability to effectively assess/evaluate and make appropriate recommendations and develop effective patients and family plan of action incorporating the family's culture, values and beliefs
* Case management
* Ability to track and effectively manage multiple tasks associated with complex case management
* Knowledge with case management record keeping process for billing Medical Assistance
* Ability to become knowledgeable of multi-disciplinary case review process
* Ability to become knowledgeable of the judicial system (parole and juvenile)
* Ability to become knowledgeable of the Oneida Integration Model of Services, a global vision of long-term patient care management, and make current and advanced adjustments based on patient and family needs
* Knowledge of AAAHC, HIPAA, OSHA, IHS, Federal, State and Local regulation, state licensing DHS 35 and DHS 75
* Knowledge of the service delivery system, the needs of the recipient groups served, or needed to be developed
* Organizational development. Well, developed interpersonal skills are required. Must be tactful and mature with the ability to work with a wide variety of personalities demonstrating objectivity, respect, courtesy, maturity, and patience.
* Ability and willingness to become knowledgeable of healthcare resources available to the Oneida population, including but not limited to programming availability through Indian Health Services and surrounding communities.

Education Requirement - Or A Closely Related Field:

Education with Equivalent Qualifications:
Essential:
* Master's Degree in Social Work
* A Combination of Experience and Education May Be Considered

Credentials:

Essential:
* Driver's License Personal & Tribal Vehicle
* Insurance Required
* 3000 hours of Master's Level Social Work with certification, required within on (1) year of employment. 
Nonessential:
* Driver Safety Training
* Social Worker License/Certified Alcohol Drug Counselor
* Basic Life Saving (BLS), within three (3) months of employment. 
* Child Abuse Reporting Policy (CARP), within ninety (90) days of employment. 
* Health Insurance Portability (HIPAA) Accountability, within thirty (30) days of employment and annually thereafter. 

Working conditions:

Essential:
* Measles mumps rubella vaccination required
* Varicella vaccine required

Experience:

Essential:
* Case management record keeping to bill Medical Assistance
* Complete a pre-employment case study prior to interview
* Two (2) years of documented Master Social Work or clinic social work
* Working with local schools, intervention, prevention
* Supervisory work with youth/adolescents

Must be willing and able to obtain additional related education and training.

Must pass a pre-employment drug screen and adhere to the Nation Drug and Alcohol-Free Workplace Policy.

A Tuberculosis (TB) Screening and/or a 2 step TB Skin Test is required within thirty (30) days of employment.

Must provide a copy of diploma/degree, license, or certification upon employment.
