LPN Case Manager – Nimble Health

Nimble Health is a growing company looking for dedicated, compassionate LPNs to join the team.
Responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating healthcare services for patients under our care. You will collaborate with healthcare professionals, patients, and families to ensure the provision of high-quality and cost-effective healthcare.

Qualifications
· Graduation from an accredited LPN program
· Current LPN license in Florida
· Previous experience in case management
· Strong assessment, critical thinking, and problem-solving skills
· Excellent communication and interpersonal skills
· Ability to work effectively in a team environment.
· Proficiency with computer systems, Salesforce experience a plus.
Salary commensurate with experience. Full-time positions are eligible to participate in company benefits including Medical, Dental, Vision, Life Insurance, LTD, AD&D, Spouse, and Dependent Life. Retirement plan (401k) with company match, paid holidays, and paid time off.
Full time 9:00am - 5:30pm on site opportunities available. Schedules are Monday – Friday between 8 am and 7 pm On-site in the Tampa office.
