Overview:

 

Under the supervision of the registered nurse, assist in the individualized care of the patient directed toward achieving the patients highest level of physical and mental wellness. Performs designated clinical procedures and non-clinical support tasks essential to nursing care. Assists in unit operational support tasks or duties. Performs duties and responsibilities in manner consistent with our mission, values, and Mercy Service Standards.

 

Safety/Risk Management: a. Demonstrates knowledge of and practices safe infection control techniques. b. Assesses safety needs and provides for a safe environment including but not limited toFall Precautions; Restraint Management; Patient Identification; c. Follow all unit/departmental Specific Policies . D. Labels and properly secures guests/patients personal property. E. Adheres to hospital wide safety regulations. F.. Performs equipment safety checks and reports faulty equipment/initiates repair. G. Follows policy/procedure for completion of incident reports. H.. Follows confidentiality and HIPPA policy/procedure. I. Complies with patient rights/regulations. J. Assists with crisis situations utilizing de-escalation techniques.

 

Coordination of Services:. A. Acquires adequate supplies to perform daily care. B. Prioritizes patient need appropriately. C. Participates in quality improvement activities that ensure and enhance quality of patient care. D. Participates in shift handoff to provide objective information to oncoming coworkers about patient needs/ treatment plan.

 

Communication Management: a. Accepts delegation of tasks and completes in a timely manner. b. Communicates with other members of the healthcare team to optimize the care of the patient. C. Gives report of patient history, status, and pertinent orders/ interventions to oncoming shift.

 

Technology: a. Performs unit specific computer functions. b. Performs clerical duties as needed including but not limited to: Answering phones, Build/ update/ break down patient charts, answer call lights, monitor safety lights. C. Utilize medical equipment including but not limited to: electronic vital signs machines, EKG, blood glucose monitoring device. D. Documents objective data in EMR

Primary Aspects: A. Observes patient mood and escalates changes to RN. B. Assists and documents the daily care of patients to include but not limited to: nutrition needs, ADL assistance, ambulation, and turning C. Takes and documents vital signs, weights, intake and output, group participation, meals, safety rounds and room checks according to policy and/ or provider orders. D. Builds rapport with patients in a therapeutic manner . E. Gathers and records patient data. F. Observes changes or unusual occurrences in patient condition and reports them immediately to the appropriate personnel. G. Participates in the admission, discharge and transfer of patients and their belongings. H.. Demonstrates flexibility regarding workload, assignments and change

Complex Aspects: A.Coordinates patient care with a licensed professional to ensure provider orders are followed b. Leads/co-leads group activities and community meetings autonomously. c.  Implements interventions along with licensed professional staff E. Reinforces safety plans once developed and/ or encourages patient to complete safety planF. Helps to set appropriate and therapeutic boundaries with patients G. Effectively communicates with patients in a therapeutic manner H. Assists a licensed professional in monitoring of patients in restraints and documents observations I. Communicates and answers basic questions with caregivers when licensed personnel are not available

Leadership Skills: A. Precepts new coworkers B. Completes necessary documentation for the orientation process


Qualifications:

Preferred Education: Prefer successful completion of a High School Diploma and/or Nurse Assistant training coursePreferred Licensure:Preferred Experience: Behavioral Health, Psych, or Direct Support ProfessionalThe physical demands described here are the representative of the minimums that must be met by an employee to perform all essential functions of the job. Most physical demands are below, plus: Minimal: lift floor to waist height of < 30 lbs (horizontally) and < 40 lbs force, balance, Frequent: hand coordination, data entry.
