The Nurse Navigator is responsible for coordinating the care of patients within the Bariatric Program. This role includes providing education and support to patients, family, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position will also work the Bariatric Coordinator to develop and support bariatric seminars and support groups.Responsibilities:
The assessment and development of an individualized plan of care through initial and periodic assessments and through communication with patient/family, physicians and other staff, the patient navigator identifies physical, psychosocial, spiritual and social needs and coordinates the development and implementation of a plan of care to meet those needs effectively, efficiently and at the appropriate level of care required.
Documentation in the Electronic Medical record Nurse Navigator documents all patient and physician interactions in the electronic medical record, including all education related encounters. The navigator utilizes documentation tools such as the psychosocial assessment form to ensure accurate and concise documentation. The navigator keeps the physician informed of treatment goals and patient milestones.
Care Coordination the Nurse Navigator coordinates the care of patients, including completing an individualized needs assessment and care plan development for each patient to address physical, psychosocial, spiritual and social needs.
1. Serves as a patient advocate
2. Serves as a facilitator for collaboration with multidisciplinary health team members
3. Focuses on patient satisfaction including timely testing and treatments; actively seeks ways to improve patient flow
4. Ensures follow-up with appropriate resources as identified
5. Informs patients of pathology results from breast biopsies.
6. Sets up surgical consults for patients based on primary physician referral.
a. Seminar development and presentation in conjunction with the Bariatric Surgeon
b. Answering potential patient questions and leading them to the appropriate personnel as necessary
c. Initial visit with the patient at the time of or within close proximity of the initial physician consult visit
d. Guiding the patient through the pre-surgical process
e. Coordinating patient consult appointments as necessary to ensure efficiency and minimal travel for the patient.
f. Obtain insurance benefit information and facilitate insurance authorization for surgery.
7. Provide each patient with the following information prior to surgery with re-inforcement postoperatively:
a. Patient education related to the specific surgical procedure
b. Pre-operative preparations
c. Post-operative expectations
d. Patient responsibilities
Patient Education Provides education to patients and families prior to initiating treatment that supplement and enhance the education that is given by the patient's oncology physician or surgeon. Provision of individualized (to include language specific) site-specific educational tools that include written, video and/or face-to-face interactions with navigator. Also responsible to review patient educational materials annually and make recommendations for changes or additions.
Communication and Multidisciplinary Management Works closely with as primary patient liaison to support the physician-patient relationship while communicating with and educating as well as coordinating patient care in an effective and timely manner (i.e. assist with appointment scheduling, medication needs, and education on specific treatments).
Data Collection: Assists Coordinator and Reviewer with development of performance metrics to measure program success. Collects and analyzes outcome data, maintains dept. statistics, records and resource files. Identifies areas of improvement and develops action plans accordingly.
Assists with community Education and Marketing of Services provides educational programs and in-services related to specific cancers to both hospital coworkers and community to increase awareness and knowledge related to diagnosis and treatment. Active and ongoing marketing plan directed at physicians and clinics to visit, educate and encourage appropriate referrals for patient navigation services in a timely fashion.
Support Group Facilitation
Facilitates support group(s) as indicated
Wears proper personal protective equipment when appropriate. Observes universal precautions consistently. Wears clothing and Health System badge consistent with dress code, and attends to personal hygiene to maintain a clean, well-groomed appearance. Meets Health System attendance standards. Performs related duties as assigned.

- Education: Graduate from an accredited Nursing School.
- Licensure: RN License in the State of Missouri.
- Experience: Minimum of three years nursing experience; bariatric surgery related experience preferred.
- Other: Critical thinking and problem-solving skills; Assessment skills to include the assessment of psychosocial needs; the ability to plan and organize time; the ability to effectively utilize resources to meet patient needs.
