General Summary of Position

Position Summary

Part of a team responsible for creating, maintaining and leading the clinical outreach program as part of the hospital's mission to
promote the public health of our regional community. An ongoing schedule of screening and educational activities that is tied to
the strategic plan of the hospital will be developed and implemented. The incumbent will also be part of a team providing
transitional care case management to patients of MSMHC experiencing challenges in chronic disease self management and
access to the healthcare system in the community. 

Key Responsibilities 

  1.  Perform Health Risk Assessments and health screenings for disease like diabetes and high blood pressure. 
  2. Administers injections for vaccinations. 
  3. Provide patient information and obtain appropriate consents for screenings and procedures performed. 
  4. Contributes to the successful implementation of the Community Health Needs Assessment plan and data collection
    needed for annual community benefit reporting. 
  5. Facilitate partnerships in the community with schools, faith-based organizations, community groups and county
    government. 
  6. Teaches evidence based classes and programs.
  7. Responsible for staffing community outreach events.
  8. Carries a caseload of patients at high risk of readmission in coordination with the RN Transitional Care Navigator.
  9. Perform all other related tasks as assigned.

Qualifications

  • ADN, BSN, MSN, or CNL from an accredited School of Nursing required.
  • 3-4 years working in the health care industry, preferably acute care required.
  • Active MD RN License or Active Compact State RN and Basic Life Support for Healthcare providers required.
