General Summary of Position
The GUIDE Care Navigator is a non-licensed member of the GUIDE team responsible for supporting patients with Dementia, families and caregivers as they navigate the healthcare system, providing information, resources and service coordination. The Navigator is a trusted resource for GUIDE patients, families, caregivers, and the care team to ensure seamless transitions of care, access to necessary services, and enhanced health outcomes. This position also supports daily practice operations including answering phones, patient registration and scheduling, DME and referral management, specialty and partner coordination, and departmental, external meeting support.


Primary Duties and Responsibilities

  • Regulatory Compliance: Utilizes Electronic Medical Record software to create and maintain accurate patient medical records and document outreach and interactions per program standard. Maintains records and data to support regulatory compliance and reporting.
  • Continuous Improvement: Maintains a mindset of continuous improvement, identifying and communicating opportunities to enhance program effectiveness. Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate.
  • Education: Completes all required GUIDE education and onboarding activities. Stays current on changes to GUIDE regulatory changes that impact departmental operations and or processes. Maintains knowledge of evidence- based practices in dementia care, care management and caregiver support. Completes mandatory education and requirements to maintain certifications.
  • May participate in on call support outside of regular hours on an as needed basis

    Minimum Qualifications
    Education

    • High School Diploma or GED required and
    • Vocational/Technical Diploma Certified Nursing Assistant, Medical Assistant, or Certified Community Health Advocate preferred

    Experience

    • 1-2 years Experience working in a medical office or ambulatory practice setting required and
    • 1-2 years Experience working as a Care Navigator, and or working with populations (geriatrics) with chronic disease and or patients with Dementia preferred

    Licenses and Certifications

    • Certified Medical Assistant (CMA)-AAMA Upon Hire preferred or
    • CNA - Certified Nursing Assistant Upon Hire preferred

    Knowledge, Skills, and Abilities

    • Strong interpersonal skills both in-person and on the phone to include verbal and written communication
    • Demonstrated knowledge of clinical procedures and medical terminology
    • Ability to work independently and as part of an interdisciplinary team
    • Exceptional organizational, problem-solving and time management skills
    • Proficient in electronic health record (EHR) and registration scheduling software
    • Experience with Microsoft Office Suite


    This position has a hiring range of $20.17 - $35.04
