Job Summary and Qualifications

1. Works with the Director of Quality and Risk Management (DQRM) to sustain a proactive, hospital-wide risk management and patient safety program, achieve annual goals, and reduce associated cost.

2. Responsible for supervising and provides administrative direction to risk management, patient safety, and infection control personnel.

3. Oversees and maintains an accurate and up-to-date incident reporting system, including risk reporting as assigned.

4.Responsible for pre-litigation and litigation activities in collaboration the DQRM; collaborates effectively with HCI and legal counsel.

5.Responsible for coordination and the overall completion of the Premium Credit program requirements so as to achieve maximum discount, including training requirements for leaders, physicians, and the Board of Trustees.

6.In coordination with the Safety Officer, assures product recall activities are managed with Materials Management, Pharmacy Director, and other department directors, including environmental safety hazards.

7.Responsible for the initiation and completion of root cause and failure mode analysis, in a timely and effective manner and in conjunction and collaboration with Quality Assurance Specialists, the CNO, and the DQRM; assists the SW VA Market as indicated.

8. Responsible to assess worksite environmental safety and evaluate hazard control activities to meet organizational safety objectives and to reduce the risk to people, property, and the environment.

9. Directs organized, appropriate responses to all complaints/grievances to meet CMS and JCAHO regulatory requirements.

10. In collaboration with the DQRM, coordinates statewide and national quality efforts as assigned to include Nursing Sensitive Indicators (NDNQI), Virginia Health Quality Center (VHQC), Anthem Q-HIP program, JCAHO National Patient Safety Goals, Leapfrog, Institutes of Healthcare Improvement (IHI) and other applicable programs. Promotes and maintains house-wide patient safety initiatives; proactively facilitates an effective and collaborative facility-wide quality culture.

11. Coordinates key patient safety metrics analyses as assigned; assures the preparation and presentation of performance reporting to key stakeholders. Assures all data collection is performed accurately and that databases and metrics reporting are appropriately maintained.

12. In collaboration with the DQRM, helps to coordinate Quality Council, Performance Improvement Council and associated task force committees; applicable Medical Staff Committees, and other committees as assigned.

13. Works in a collaborative manner with all Departmental Leaders, Clinical Staff, Case Management, Employee Health, Clinical Documentation Specialists, Physicians, and other Allied Health Professionals. Assists the DQRM with communication and education regarding risk management and patient safety activities in the organization and the community, and serves as a resource.

14. Works to expand knowledge of risk management and patient safety by bringing new ideas and concepts to LGHM.

15. Other duties as assigned. 

What qualifications you will need
  • Registered Nurse with Bachelors Degree or other related health care degree from an accredited institution required
  • At least four (4) years healthcare experience in risk management, performance improvement, safety, and/or critical care or other related clinical area. Must possess exemplary critical thinking skills, strategic decision making skills, problem solving skills, and initiative. Computer skills necessary.
