The Holyoke Visiting Nurses Association, Inc. is looking to hire an Intake Coordinator working Per Diem! Some of the main functions of this position would include the following tasks of obtaining patient information from referral source including, but not limited to: diagnosis, MD order to assess, teach, or provide direct care, expected frequently, etc.# In addition, verifying MD licenses and updating data base periodically. Perform initial assessment of patient needs for home care services based on referral source information, to determine whether Medicare/Medicaid or other third party payer criteria is met. Obtain initial physician orders, as well as follow VNS policies and procedures for the referral process, i.e., delay in initiating services. Establish and maintain positive relations with all customers,# external as well as internal. Discuss home health services with referral sources, answering questions and identifying services available for specifically referred patients.#Also, offer alternative settings when home care is not appropriate, for example: outpatient mental health providers, skilled nursing facilities. Clearly communicate patient needs to appropriate individuals including supervisors, Referral Center, and clinical staff, when indicated. Conduct insurance verification and pre-authorization of services as a back-up to the Benefits Specialist, as requested by supervisor. Input patient referrals, interoffice referrals, cancellations and other information into Clinical Link and other computer systems. Co-sign LPN orders and perform other similar or related duties as responsibilities necessitate or as assigned. # GENERAL DUTIES: Actively perform job duties in a manner that will promote and enhance the organization#s mission of providing high quality services to all of its customers. Conduct all activities in full compliance with all relevant laws and with regulatory requirements affecting business of the agency.# Comply with the agency Code of Ethics, Code of Conduct and other policies as set forth by the agency. Participate in the agency process improvement plan through involvement in process improvement committees as assigned by supervisor, identifying areas for improvement in job functions and reporting these to supervisor, and performing and recording quality control tasks as assigned. To act in a courteous and respectful manner of fellow employees, patients and customers of the Agency. Establish and maintain positive interpersonal relationships that promote a willingness to work with others cooperatively and collaboratively. Adhere to agency policies and work standards on confidential information.
