The Holyoke Visiting Nurses Association, Inc. is looking to hire a Hospice and VNA Systems RN Case Manager with a $10,000 sign on bonus for this role! Some of the duties and responsibilities include taking full and ultimate responsibility for the quality of nursing care she/he provides to individuals and groups. Included in such responsibility is health maintenance, teaching, counseling, collaborative planning and restoration of optimal functioning and comfort or for the dignified death of those they serve. A registered nurse, within the parameters of his/her generic and continuing education and experience, may delegate nursing activities to the registered nurses and/or health care personnel, provided, that the delegating registered nurse shall bear full and ultimate responsibility for: (1) Making an appropriate assignment (2) Properly and adequately teaching, directing and supervising the delegate; and (3) The outcomes of that delegation. A registered nurse shall act, within his/her generic and continuing education and experience to: (a) Systematically assess health status of individuals and groups and record the related health data; (b) Analyze and interpret said recorded data; and make informed judgments there from as to the specific problems and elements of nursing care mandated by a particular situation; (c) Plan and implement nursing intervention which includes all appropriate elements of nursing care, prescribed medical or other therapeutic regimens mandated by the particular situation, scientific principles, recent advancements and current knowledge in the field; (d) Provide and coordinate health teaching required by individuals, families and groups so as to maintain the optimal possible level of health; (e) Evaluate outcomes of nursing intervention, and initiate change when appropriate; (f) Collaborate, communicate and cooperate as appropriate with other health care providers to ensure quality and continuity of care; (g) Serve as patient advocate, within the limits of the law. Registered Nurse System Case Manager bears full and ultimate responsibility for quality of nursing care provided to a group of patients based on desired outcomes consistent with evidence based guidelines and quality care standards. Ultimately responsible for the design and implementation of the plan of care and initiating changes in the plan of care when appropriate with the interdisciplinary team. Accountable for clinical and financial patient outcomes. Proactively facilitate coordination of services and utilization of community resources with interdisciplinary team. Coordinate care and communication between team members and physicians to ensure appropriateness of care. Plan for and facilitate appropriate discharge from services with the interdisciplinary team. The RN system case manager is responsible for the delegation of patient activities to other members of the interdisciplinary team provided that the nurse system case manager shall bear full and ultimate responsibility for making an appropriate assignment. Provide skilled nursing care to patients in the community and inpatient in accordance with agency policies and procedures. Provide care in a manner, which promotes quality, continuity and safety for the patient. Provide professional nursing care utilizing all elements of the nursing process. Perform initial and ongoing assessment of the impact of the medical or terminal diagnosis on a patient#s physical, functional, emotional and spiritual status including verbal and non-verbal communication patterns. Refer patients to community agencies, health providers and other agency services as appropriate to meet the needs of the patient. Instruct patients and caregivers in symptom management and end of life care. Evaluate patient#s progress towards goals, reporting changes to the interdisciplinary team and physician.# Revise the plan of care in collaboration with the interdisciplinary team. Provide on call coverage for off hour shifts. Respond promptly to on call calls, providing telephone triage, admission, routine and as needed nursing visits. Provide thorough and timely reports to the supervisor and/or interdisciplinary team related to on call activities.# Complete, maintain and submit accurate and timely documentation and relevant clinical notes regarding the client#s condition and care and treatment provided. Promote a safe environment for the client, family and hospice and VNA staff. Seek and obtain sufficient information necessary to work effectively with providers in inpatient facilities. Represent Holyoke VNA, Hospice Life Care in a professional manner to the community and other health care providers. Maintain a working knowledge of community resources. Respect patients# rights as stated in the agency policies and client Bill of Rights. Supervise home health aides and delegate responsibilities appropriately. Attend and participate in interdisciplinary team meetings. Attend and participate in #services and mandatory education programs. Demonstrate knowledge and skills related to clinical nursing practice, and effective symptom management and palliation of pain. Initiate communication with the physician to facilitate recommendations for symptom and pain management..# Demonstrate knowledge and understanding of the psychosocial dynamics operative in end of life and palliative care. Demonstrate knowledge of the dying process as it relates to patients, families and society as well as self. Include social, cultural, and age specific considerations in the development of the care plan. Assess for pathological grief and refer appropriately. Maintain a working knowledge of both Quality and Patient # Family Satisfaction Measures for both VNA # Hospice (OASIS, HIS, CAHPS, HHCAHPS) and work collaboratively with patients, families, and agency personnel to improve quality scores. **HOT JOB**
