Our Oncology team is seeking a Social Worker to evaluate#and respond#to the needs of individuals and/or their families. They provide#care based on the documented clinical assessment of the patient#s needs, which includes a culturally sensitive assessment of biophysical, psycho-social, environmental educational,#and discharge planning#factors. Will also develop#clinical formulation that is based on assessment data and grounded in clinically sound theory that directs treatment toward individualized, solution-focused interventions.# Must be knowledgeable about disease, treatment, and the psycho-social implications for individual patients and their families. Facilitates#a smooth transition for the patient through the continuum of care and appropriately access hospital, community and government resources, and assist the patient and families with end of life issues including hospice services coordination and advanced directive documents.# REQUIREMENTS: Masters of Social Work from a program accredited by the Council on Social Work Education (CSWE). #Current Massachusetts LICSW#or LCSW.# Three years of medical social work experience.
