Implements the comprehensive plan of care by providing direct nursing care within a shift for individual patients. Initiates Interdisciplinary Plan of Care or Guidelines assists the Patient Care Coordinator in devising a comprehensive plan of care and evaluates patient responses to implemented care. Assists patients in achieving their long-term goals by establishing and meeting short-term goals and providing education to meet their discharge needs. Other duties as assigned. JOB REQUIREMENTS: Experience No experience necessarily required but is dependent upon unit shift current staffing situations; may require specific clinical experience. Education Associate degree in Nursing is required. Licensure Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact) or permit to practice as a graduate nurse. American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days. American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) is preferred. JOB STANDARDS: -Collaborates with Patient Care Coordinator and health care team to establish appropriate interdisciplinary plan of care for individual patients. -Assesses and analyzes health status data and documents accurately. -Delivers nursing intervention based on assessments in a timely manner to maximize patient outcomes. -Evaluates the patients responses to implemented care. -Delegates and coordinates patient care activities with the appropriate members of the health care team to achieve expected outcomes. -Implements and documents progression toward the outcomes established for the plan of care. -Evaluates discharge needs and implements appropriate referrals and other interventions. -Demonstrates commitment to professional nursing through involvement in decisions impacting clinical practice and using established standards to measure assess and improve performance. -Supports change and facilitates implementation of change. -Utilizes therapeutic communication to enhance patient teaching and develop effective goals. -Assists in the orientation and development of nursing personnel/students as appropriate.
