Job Summary:
The Social Worker is responsible for assessing and meeting the needs of patients and families with regard to psychosocial aspects of the episode of care. The Social Worker functions as a member of the Care Coordination team to assist patients and families in appropriate and timely discharge from the inpatient setting by facilitating the transition to alternative settings or home. The Social Worker is the team member accountable for addressing counseling needs of the patient/family as they relate to coordination of care.

Job Essentials:
- Minimum of 2 years of nursing experience required.
- Health Care or Case Management experience required.
Education Requirements:
- BSN required
License/Certification/Registry:
- NJ State Registered Nurse License
- BLS Certification
