We are seeking a PRN RN Home Health Care Manager to join our team in the Cedar Hills, Pantego, Fort Worth, Keller, Alliance, and Denton areas. In this role, you will be responsible for coordinating and providing high-quality, compassionate care to patients in their homes.

Responsibilities:
- Conduct initial assessments and develop individualized care plans for patients
- Perform skilled nursing care, including wound care, medication administration, and monitoring vital signs
- Coordinate care with other members of the healthcare team, such as physicians, therapists, and social workers
- Educate patients and their families on their condition and how to manage it at home
- Evaluate and update care plans as needed to ensure optimal patient outcomes
- Maintain accurate and up-to-date patient records

Qualifications:
- Current RN license in Texas
- Minimum of 1 year of clinical experience, preferably in home health or a similar setting
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Must have reliable transportation and be willing to travel to patient homes within the designated service area
