Job Title: Home Health LVN (Licensed Vocational Nurse) Part Time for the FTW, Keller, Denton, Pantego area.

Job Summary:
Our company is seeking an experienced and compassionate Home Health LVN to provide professional care and oversight to our clients who need medical support in their home. The ideal candidate for this position will have excellent communication skills and a passion for helping patients to maintain their independence.

Responsibilities:
• Evaluate and coordinate clients' home healthcare needs
• Administer medications, wound care, injections, and other necessary medical treatments as prescribed by the physician
• Monitor patients' vital signs, observe and report any changes to the physician
• Educate patients and their families on proper healthcare procedures and assist them with any questions or concerns they may have
• Collaborate with other healthcare professionals, including physicians, social workers, physical therapists, and other caregivers
• Update clients' medical records, progress notes, and care plans
• Maintain a high level of professionalism, following HIPAA regulations and ensuring clients' privacy and confidentiality

Requirements:
• Active LVN license in good standing in the state of employment
• Minimum of 1 year of nursing experience in home health and/or acute care settings
• Strong organizational and multitasking skills
• Excellent verbal and written communication skills
• Ability to work independently under minimal supervision
• Compassion and empathy for patients and their families

Work Schedule:
This is a part-time position, with set hours Monday - Friday and 1 week of oncall every 5-6 week.

Physical Demands:
The Home Health LVN must be able to lift and move patients, stand and walk for prolonged periods, and perform other physical tasks required when delivering healthcare in a patient's home.
