**Overview**

Dignity Health Mercy San Juan Medical Center is a 370-bed not-for-profit Level 2 Trauma Center located in Carmichael California serving the areas of north Sacramento County and south Placer County for more than 50 years. Mercy San Juan Medical Center has received recognition for being a Comprehensive Stroke Center and Center of Excellence for Bariatric Surgery along with Certificates of Excellence in Perinatal Care Hip- and Knee- Replacement.

**Responsibilities**

The Stroke Nurse Navigator is responsible for evaluating and improving care for Mercy San Juan Medical Center's stroke patients. The Navigator has a pivotal role in the development of high impact care activities and initiatives for the stroke patient population, their families, and the greater community and serves as a resource to neurology department stroke committees and the Joint Commission certified Comprehensive Stroke program at Mercy San Juan Medical Center.

Additional responsibilities include:

Organizing relevant stroke program activities through inter-departmental collaboration, service coordination and patient follow up and projects

Representing the Comprehensive Stroke Center at community and professional groups and events

Assisting with new program development and data collection and extraction as directed by the Neuroscience Service Line Director

**Qualifications**

**Minimum:**

Current CA RN licensure and AHA BLS

3 - 5 years of full time experience as a RN with critical thinking and working with high-acuity patient populations

Experience in providing formal and informal physician and nurse education

BSN

Computer skills utilized in the healthcare environment

**Preferred:**

Experience in community care and chronic disease management

**Pay Range**

$64.64 - $83.51 /hour
