**Job Summary**

The Case Manager is accountable for coordinating the efficient and effective operations of case management and utilization review. The Case Manager is responsible for timely discharge planning, meeting LOS benchmarks, and positive working relationships with physicians, payers, Avera Sacred Heart Hospital staff and transferring/receiving facilities

**Education and/or Experience**

Bachelor's degree in nursing preferred.

Must have current S. D. registered nurse license, if residing in S. D. or a "multi-state privilege" to practice in SD if residing in compact state.

BLS Certification.

**Work Schedule**

Varied eight-hour shifts, every 3rd Saturday; PRN (as needed)

**Pay**

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$29.50 - $44.25
