Summary The Iowa City VA Health Care System (ICVAHCS) is currently recruiting for one (1) Full-Time Registered Nurse- Assistant Nurse Manager (ANM) - Home Telehealth Health position. ICVAHCS is in Veterans Integrated Service Network (VISN) 23. The ANM Home Health position falls under the Nursing, Connected Care service line. This position is only open to current employees at the Iowa City VA facility. Responsibilities The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. The position provides direct and indirect nursing care to patients and families who are utilizing the Home Telehealth modalities. The ANM is responsible for adhering to hospital policies/procedures and standards of practice. This nurse will demonstrate leadership within the department and serve as a role model when functioning as a team leader, under the supervision of the Telehealth Clinic Manager. The ANM is a professional who provides oversight to the HT program in the day-to-day operations of the Care Coordination Home Telehealth program. In addition, this individual coordinate care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. The ANM works closely with program staff, the primary care provider (or specialty care providers), other healthcare professionals, team members, clinics, community agencies, and medical center leadership. The incumbent provides professional assessment, coordination and planning of multiple health services; advocates for the Veteran to assure that necessary clinical services are received, and that progress is being made to meet the goals established by the ANM and the Veteran. The ANM provides ongoing evaluation and care management services provided by program staff. This includes, but is not limited to the following: 1. Serving as local, VISN, and National contact for specific types of communication related to RPM-HT. 2. Taking a direct or indirect supervisory role in reviewing and evaluating the work performance of other RPM-HT employees, making assignments (such as coverage for patients in the event of staff absences), ensuring staff competency, and facilitating or performing peer reviews. 3. Providing guidance by establishing and maintaining teamwork among staff. 4. Ensuring awareness of National, VISN, and facility level policies and procedures related to the continuity of care during emergency and disaster events. 5. Participating in Local, VISN, and National Committees. 6. Serving as a point of contact (POC) for the RPM-HT program of Conditions of Participation (COP), which includes maintaining compliance, addressing deficiencies, and ensuring that the program meets the COP. 7. Providing education to Providers/staff regarding HT. 8. Orientation of new staff in the business, clinical, and technical program processes. In accordance with 38 U. S. C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant a
