Description
The LPN/LVN - Hospice is responsible for providing care, comfort and support to terminally ill patients. The LPN/LVN - Hospice assumes the responsibility and accountability for the delivery of hospice patient care in various settings. The LPN/LVN demonstrates the ability to make clinical judgments in an effective and efficient manner with supervision. The LPN/LVN will also demonstrate critical thinking and performance ability in the coordination of patient care.
Essential Duties and Responsibilities:
Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
Identifies and prioritizes actions based on patient care requirements
Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard
Provides a safe, comfortable and therapeutic end-of-life patient environment
Provides patient and/or family education, guidance and support
Documents the patient's response to nursing and other interventions
Uses clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit including establishing priorities
Monitors safety and effectiveness of the environment and equipment
Advises grieving family members on coping techniques and community resources - Performs other duties as assigned/necessary
Minimum Requirements:
Current Licensed Practical Nurse/Licensed Vocational Nurse for the state in which the nurse practices
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age
