LPN Nurse
Job Description
The primary functions of an LPN include assisting with assessment, planning, implementing, and evaluating the care of all patients, regardless of age, during their shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides person centered care and services. Responsibilities include taking the patient's vital signs such as respiration, temperature, blood pressure, and heart rate, and reporting changes in the patient's/residents conditions. Practices ethically and professionally within their scope; advocates for patients. Maintains clinical competence specific to their care area including organization required education within the stated timeframe. Coordinates care with the interdisciplinary team. Documents appropriately and thoroughly in EMR/Point Click Care. Keeps other nursing personnel and responsible RN informed regarding changes in condition of patients/residents and/or other pertinent observations. Participates in unit councils and staff meetings.

Work Site
This is a fully on-site position in Ithaca, New York.
Work Environment
On site - Evenings and overnights available. 32 hours per week. Every other weekend and holiday required. No RTO during initial assignment allowed
