Summary of Position
* Assist the Manager, Nursing in the management and education of care team members providing professional nursing care to
patients. Proactively support efforts to ensure that standards for quality of care, patient satisfaction, and patient safety are
maintained, as established by AdvantageCare Physicians including best practices, regulatory requirements, and clinical
workflows.
* Serve as a leader, role model and liaison for the care team, ensuring quality patient-centric care at the medical offices.
* Provide educational support for all care team activities.
Principal Accountabilities
* Assist in managing the daily operations of the medical office care teams including, ensuring that standards for quality
of care, patient satisfaction, and patient safety are maintained, as established by AdvantageCare Physicians (ACPNY).
* Assist in screening and selection of applicants and providing general orientation to newly hired team members.
* Assist in providing work direction, assigning schedules, conducting on-the-job training, monitoring/ evaluating care
team performance, and recommending personnel actions.
* Assist in the interpretation of and compliance with Human Resources, ACPNY and departmental policies and
procedures.
* Ensure employees understand and comply with local, state, federal and regulatory standards, as they apply to
respective job functions.
* As a member of the interdisciplinary healthcare team, collaborates with providers and others on the clinical care
team to formulate and implement comprehensive patient centered plans of care to achieve expected outcomes.
* Interface with physicians and all staff members as it relates to care team process.
* Act as the "Manager, Nursing" in his/her absence and cover other medical offices in absence of the clinical lead.
* Assist in ensuring compliance with all infection control policies, procedures and OSHA (Occupational, Safety and
Health Act) standards. Participate in program evaluation and quality improvement activities; actively seek input to
support patient care and outcomes. Ensure all HEDIS (Healthcare Effectiveness Data and Information Set), HCC
(Hierarchical Condition Categories) and PCMH (Patient-Centered Medical Home) measures are completed in the
required time frame.
* Maintain HIPAA compliance and report any breaches to the Manager, Nursing and the Practice Administrator.
* Collaborate with the Quality team to identify areas in need of improvement as well as sharing of Best Practices.
* Collect all metrics related to patient outcomes and review findings with the Manager, Nursing.
* Participate in quality improvement activities; actively seek input to support patient care and outcomes.
* Work in a clinical capacity, providing back-up when appropriate. Understand and support practice of other team
members and refer patients appropriately.
* Assist providers with procedures as needed.
* Provide surgical coordination and oversight of the Endoscopy specialty suites.
* Assist the Manager, Nursing in conducting monthly staff meetings.
* Assist with daily huddles and in-basket messages and as necessary address delinquencies with appropriate staff.
* Collaborate with Case Management team members on complex cases as needed.
* Assist in promoting employee engagement and wellness, shared decision making, and collaboration with all members
of the healthcare team. Collaborate on the development of staff related initiatives to show gratitude and celebrate
staff recognition.
* Ensure nursing practices of the care team as well as own practices are in accordance with NYS Department of
Education nursing standards.
* Collaborate with Manager, Nursing on employee performance evaluation, issues and improvement plans for Nursing
Staff.
* Support ongoing education within the practice for all clinical staff as well as
