Job Description
Must be located in Northern Virginia.

Approximately 50-75% of field based travel required once COVID restrictions are lifted.

The Diabetic Educator will work collaboratively with the commercial and Medicare Primary Case Managers on high risk/high cost members who are identified with:
-Non-adherence to Diabetic medication regime
-Support needs on nutrition
-Gaps in self-management

Medication adherence and dietary compliance related to Diabetes can produce barriers to effective management of the member's health issues or chronic condition(s). The Diabetic Educator will address identified issues through an evaluation of gaps and discussion with the member/family/caregiver which may include:
-Diet and lifestyle modification
-Medication adherence
-Family or Community support systems
-Barriers to self-management of health issues/chronic condition(s)

-Participate in all Multi-Disciplinary Team Case Conferences and Huddles
-Collaborate with the Primary Case Manager
-Provide consultative review and recommendations to the Primary CM when indicated
-Outreach to the member when indicated and assist with the following:
o Identify gaps in self-management of chronic condition(s)
o Educate on lifestyle modification
o Provide nutrition counseling
o Educate on medication adherence
o Identify family and social supports
-Build and execute group visits for education on nutrition and lifestyle modification
-Provide Education to the JV-MDCT team on opportunities, JV and Community resources for Diabetic members and red flags for referral
-Final closure of referral will be determined through mutual discussion between Diabetic Health Educator and Case Manager, and documented in appropriate system (ATV, MedCompass)

Required Qualifications
- CDE (Certified Diabetes Educator) or CDCES (Certified Diabetes Care and Education Specialist) required or must currently be working towards certification and obtain certification within 90 days of hire
- Strong verbal and written communication skills and the ability to communicate effectively with all levels of management required

Preferred Qualifications
- RN with current unrestricted state licensure highly preferred
- 5+ years of managed care experience preferred
- 3+ years of complex case work experience preferred
- Medicare case management experience preferred
- Systems experience preferred: ATV, MedCompass, ASD, HRP, Microsoft Outlook

- Associate's degree required
- Bachelor's degree preferred
