Job Description
Applicants must reside in Maricopa County preferably on the east side.

Once COVID restrictions are lifted, this role will be part time based in external clinics and part time from home.

Schedule: Monday-Friday 8am-5pm

Mercy Care RBHA care manager utilizes critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management tools and resources.

Mercy Care RBHA care manager utilizes knowledge of program requirements, network and community resources to facilitate appropriate physical and behavioral healthcare and social services for members through collaboration with internal and external

Required Qualifications
-2+ years of experience in behavioral health or social services required
-A minimum of a Bachelor's degree licensed master level clinician required

COVID Requirements

Preferred Qualifications
- Case management and discharge planning experience preferred
- Managed Care experience preferred
- Experience with SMI population preferred
