Summary:

This is nursing work in the care and treatment of individuals with a mental health (MH) diagnosis, which may include physical health issues, and includes a focus on providing wellness education and connection with physical health providers. The Wellness Nurse reports directly to the Program Director.

This position is part of our Mental Health (MH) Outpatient Treatment Services. This is a great opportunity for recent graduates or seasoned professionals looking for a nursing position outside of a traditional medical office, nursing home, or hospital setting.

Full-Time (40 hrs./wk.) Monday to Friday - 8:00 AM to 4:30 PM. No evenings, weekends, on-call, or mandated overtime.

Wage Information:

LPN: $22.85 - $28 per hour. RN: $27 - 33 per hour.

Job Description:

  • Educates and provides wellness coaching to individuals related to medication, physical health, behavioral health, nutrition, tobacco cessation, and self-care. 
  • Evaluates, observes, and assesses physical health conditions, particularly for individuals with extensive comorbid physical health/behavioral health (PH/BH) conditions.
  • Assists individuals in developing an individualized wellness plan.
  • Analyzes individual and aggregate PH/BH care utilization data from PH/BH Managed Care Organizations (MCO) on identified individuals.
  • Develops wellness materials for individuals on identifying and managing chronic health conditions.
  • Provides assistance to physicians/Certified Registered Nurse Practitioner (CRNP) as needed.
  • Monitors and documents vitals.
  • Administers injections per physician orders.
  • Develops and implements programs around adherence to medication and medical appointments.
  • Assists and coaches individuals to advocate for themselves in meetings with health care providers.
  • When necessary, accompanies individuals/Targeted Case Manager (TCM) to medical appointments to ensure they are getting optimal PH services and provides follow-up consultation and monitoring to ensure optimal compliance with PH providers' recommendations.
  • Creates awareness of PH/BH issues throughout the behavioral health treatment team.
  • Works with and coordinates care with pharmacy, physicians, and other health care providers.
  • Convenes team meetings for individuals with complex PH/BH conditions to ensure coordination of care as needed.
  • Assists/manages medication refills, as needed.
  • Completes pill counts on an established schedule if required by program.
  • Completes and facilitates medication disposal, as needed.
  • Completes medication records (Medication Account Records, Physician Order Sheets, Nursing Tracking Forms etc.) as per established schedule.
  • Provides First Aid/Acute care as needed.
  • Provides nursing care in accordance with the Nurse Practice Act and legal responsibilities as defined by the State Board of Nursing. 
  • Knowledge of the laws and regulations pertaining to nursing.
  • Knowledge of the various methods, technology, and procedures used in nursing.
  • Knowledge of the uses and effects of psychotropic drugs and narcotics, safety practices and location security.
  • Knowledge of Mental Health/Intellectual and/or Developmental Disability (MH/IDD) diagnostic categories.
  • Knowledge of common illnesses, diseases, disabilities, injuries, and nutrition.
  • Ability to collect data and document in the individual's record according to policy and procedures, from individual's admission and throughout the stay and communicate relevant information to the program director. 
     

Qualifications:

This position requires one of the following combinations of education and experience:

  • A valid Licensed Practical Nurse (LPN) license; or
  • a valid Registered Nurse (RN) license in the state of Pennsylvania.

Additional requirements include:  

  • Must have a valid driver's license, good driving record, and access to a reliable vehicle to attend trainings, meetings.
