Seeking an Acute Care Certified Nurse Practitioner (or experienced PA) to join a team of highly performance Hospitalist Team with focus on quality and patient centered care in a state of art facility.

You would be required to work side-by-side with a team of IM or FM board certified physician hospitalists and acute care certified nurse practitioners in an acute care setting at a critical access facility with multiple specialists services. Hospitalists provide care for all inpatients except Peds and OB GYN.

Majority of work would involve co-management of surgical patients along with a mix of medical cases. The job offers opportunity to grow in your career with ongoing mentoring as needed while independently managing your own panel of patients.

Opportunities to enhance your skills by use of bedside ultrasound and other technology in a state of art facility.

A typical work week comprises of 20 hours with short and long shifts. One weekend per 4 weeks coverage for rounding. Holiday coverage shared with three other nurse practitioners. This is a part-time position with opportunity to increase hours to cover vacation and CME.

Certificate in Acute care and unrestricted NP license in MN a must for applying for this job.

Shift Rotation: Day Rotation with 1 in 4 weekends. Long shifts are 7 A – 7P. Short shifts are 7 A – 5P. Weekends are 7 A – 5 P.

Position Description

APP Hospitalists provide services to inpatient hospital care. They admit, care for and discharge patients. They perform procedures as appropriate with their privileges granted per the credentialing process.

APP Hospitalists responsibilities include rounding on and admitting patients, participating in team meetings, being aware of all assigned patient's treatment plans, documenting findings in patient charts, assessing and managing medications, ordering and reviewing lab tests or other diagnostic tests. The extent of the involvement in the assessment and implementation of treatment will depend on the complexity and acuity of the patient's condition and the training, experience, and preparation of the physician assistant and/or nurse practitioner as determined by the hospitalist or supervising physician.

ESSENTIAL RESPONSIBILITIES
  • To provide comprehensive health care for assigned hospitalized adult medicine patients admitted to the hospitalist services. The patient population includes patients 17 years of age or older.

  • Care for patients with acute and chronic diseases by managing therapeutic regimens, prescribing medications and performing other studies and activities deemed appropriate for role and licensure.
  • New and follow-up patients will be seen and evaluated by the APP Hospitalist. Physician input will be obtained as necessary as described in the APP Hospitalist guidelines.
  • Facilitates the referral process for patients requiring multidisciplinary evaluations. Collaboratively work with the multidisciplinary team.
  • Patients evaluated by the APP Hospitalist include those patients admitted to the medical services under the hospitalist program and those patients on various other services for whom hospitalist services have been requested.
  • Performs disposition planning and medication reconciliation.
  • Demonstrates a Can-Do Spirit when other duties are assigned.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS
  • May be exposed to communicable and infectious diseases. Possible injury from patients during excited phases of illness and cuts from sharp instruments.
  • Possibility of strains due to lifting patients or handling heavy equipment.

Qualifications

POSITION QUALIFICATIONS
  • Education and Experience:
    • Completion of an accredited Nurse Practitioner and/or Physician Assistant Program.

    • Previous experience as APP hospitalist preferred. 
  • License/Certificates:
    • Minnesota Nurse Practitioner and/or Physician Assistant licensure. American Heart Association CPR and ACLS certification. DEA registration.

    • Certified in Acute Care training preference for experience in Acute Care or hospital medicine.
    • Successful completion of the credentialing process at CRMC.
  • Special Skills and Aptitudes:

Compensation and Benefits

The pay range listed above is the estimated starting pay range for this role. Please keep in mind that the actual pay rate offered could be higher for some candidates depending on many factors including each individual employee's experience level and/or preferred education or certifications.
