Salary Scale: $23.83-34.97 (BSW)
Salary Scale: $30.51-38.95 (MSW) POSITION SUMMARY Assesses, plans, implements, and evaluates the care needs of patients throughout the organization. Collaborates with physicians, nursing staff, and other care team members to ensure appropriate services are provided to meet the needs of the patients. Coordinates the discharge planning activities to assure that the patient has continuity of care after hospital discharge. Essential Functions:
  • Demonstrates knowledge and skills necessary to assist in providing care appropriate to the ages of the patients served within the organization.
  • Maintains performance in compliance with State and Federal laws, and consistent with high standards of business and professional ethics.
  • Knowledge of HMOs, MCOs, and insurance industry.
  • Analyses insurance, governmental, and accrediting agency standards to determine criteria concerning admissions, treatment, and length of stay of patients.
  • Provides outreach and referrals for patients throughout the organization.
  • Reviews inpatient medical records and confers with medical and nursing personnel and other professional staff to determine legitimacy of treatment and length of stay.
  • Assists with abstracting, organizing, and reporting of data for various regulatory agencies.
  • Coordinates discharge planning; makes referrals to appropriate facilities, home care agencies, etc. as needed. Collaborates with health care team, patient and family to develop the discharge plan.
  • Makes recommendations for level of care changes and coordinates transfer of patients to different levels of care and to other facilities. Explains facts to patients and families and assures that correct forms are signed and documentation is complete and correct.
  • Serves as the team coordinator between all members of the health care team and the patient/family. Coordinates care needs of the patient including community resources when appropriate.
  • Assists with Level 1 PASRR notifications and assists in completing Level 2 PASRR evaluations when needed.
  • Maintains current community and other resource data.
  • Assures a coordinated interdisciplinary effort that reflects individual needs within the context of both the family and the health care system.
  • Coordinates patient care conferences to develop treatment plan specific to the patient's needs and goals.
  • Maintains effective communication with medical staff, fellow staff and all levels of supervision.
  • Maintains professional growth and development.
  • Participates in staff meetings and hospital wide in-service programs as required.
  • Actively supports and participates in the continuous performance improvement efforts.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
  • Detail oriented, self motivated, must be able to work independently without close supervision, be a self-starter.
  • Graduate of an accredited Social Work program or other health care related field. Licensure in the State of Iowa. Related education/experience in Case Management acceptable.
  • BCLS certification.
  • Ability to read and communicate effectively in English
  • Strong written and verbal skills.
  • Must be able to communicate with all types of people from all socio-economic status in a non-judgmental approach.

WORKING CONDITIONS:

Typical working conditions include contact with patients/families/hospital staff and physicians. Interaction with others may be constant and interruptive at times. Time is also spent sitting at a desk for long periods of time, working on the computer, or talking on the phone.
