NursingWith Banner's Home Infusion Therapy (HIT) service, an integrative team of Clinical Pharmacists and Nurses, Technicians and Pharmacy Service Representatives work directly with patients and caregivers to create a safe and effective option for care in the comfort and privacy of their homes. The pharmacy infusion team coordinates with physicians, discharge planners and other healthcare professionals to provide a consolidated approach to home infusion needs. Home Infusion Therapy is appropriate for all patients from infants to adults allowing freedom and better quality of life throughout a course of treatment. The dedicated and experienced infusion team is focused on providing the highest quality IV service available in the community and customer service is an ongoing department objective. This is HITs commitment to our patients and families. Some of the therapies provided are Antibiotics, hydration, steroids, TPN, IVIG, Inotropes, Antiemetics and other infusion therapies.
As a Registered Nurse- Home Infusion, you will join the Banner Home Infusion Pharmacy/Nursing Team providing exceptional care in our community. You will enjoy an autonomous practice focusing on teaching about the safe administration of IV medications and self-management of such therapies within a patient's home environment. The Home Infusion profession allows for time to create relationships with your patients and their caregivers in addition to some flexibility over your schedule that is not possible in a hospital setting. You will enjoy working in a sub-assigned territory with a small travel distance. Banner Home Infusion nursing offers an opportunity to treat short and long term as well as simple and complex cases such as Antibiotic infusions and TPN and Inotrope therapies. The Infusion Team is constantly expanding in collaboration with Banner community hospitals and health centers. POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patien
