Overview
Within established enterprise policies and procedures, the Medication Safety Pharmacist is responsible for improving medication safety in the market(s) through the development of and implementation of initiatives that drive a safer medication use system. These initiatives may be realized by monitoring medication-safety related metrics and medication event analyses, as well as by completing proactive risk assessments. This position serves as the market(s) resource for providing medication safety expertise internally and externally, and provides steadfast leadership in promoting an improved culture of safety. Additional responsibilities include education of healthcare professionals and precepting of learners, representation on regional and market-level committees, leading market-level committees, and collaboration on department and interprofessional initiatives. A commitment to customer satisfaction, efficient/effective use of resources, teamwork, and innovation is required.
Education, Training, and Experience
Pharm.D., M.S., or B.S. Degree with equivalent clinical practice experience required.
Two years relevant pharmacy experience required.
