Description

+ Provides timely review of Start of Care, Resumption of Care and Recertifications. Utilizes CRC tool to provide clinical feedback related to OASIS accuracy.

+ Accurately codes chart based on primary and secondary reasons for home care based on Chart review. Advances 485 timely.

+ Provides timely feedback and education to clinicians regarding OASIS, care plans, wound identification and appropriate orders.

+ Identifies clinical trends and reports to education, clinical manager to develop team member educational programs.

Required:

1. Bachelor degree in Nursing preferred

2. 3+ years of experience in home health Nursing in a Medicare certified agency

3. Current state licensure as a Registered Nurse and BLS certification required.

1. OASIS and Coding Certification
