**Key Responsibilities:**
- Oversee the daily operations and management of the care team, ensuring high-quality service delivery.
- Develop and implement individual care plans tailored to meet the specific needs of clients.
- Supervise, mentor, and support care managers to enhance their skills and effectiveness.
- Facilitate communication and collaboration between clients, families, and healthcare providers.
- Monitor and evaluate care plans to ensure they are effectively meeting the needs of clients.
- Lead regular team meetings to discuss case updates, challenges, and improvement strategies.

**Qualifications:**
- Minimum of 5 years of experience in care management or a related healthcare field.
- Proven leadership experience with the ability to motivate and guide a team.
- Strong understanding of care management practices and healthcare regulations.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced, dynamic environment.
- Certification in care management is a plus.
