The Acadian Health Division is looking to hire a full-time Registered Nurse in Eau Claire, WI. 

The selected candidate will primarily be responsible for the care and management of the patients enrolled in a partnered Acadian Health Program. Responsibilities include but are not limited to environmental assessments, comprehensive clinical assessments, interaction and treatment of patients, including facilitating continuity of care by interacting with other members of their interdisciplinary care team. Selected candidates will be responsible for participating in medical research and treatment modalities as directed by the Medical Director; as well as other operational support functions as assigned. Additional duties may include leadership roles and training/mentoring as assigned. 

 

RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO: 

  • Conduct comprehensive physical/environmental assessments based on patient-specific needs 

  • Provide non-emergent acute clinical care in a non-traditional setting 

  • Work as an extension of virtual care teams, assisting with patient care and coordination under personalized-patient medical direction 

  • Support and monitor the patient's care plan, identifying and reporting any deviations from the prescribed patient care plan

  • Engage with patients and their care teams through virtual conferencing and in-person home visits 

  • Provide disease-specific education to the patient and/or caregivers 

  • Collaborate with various agencies and Acadian Health team members to maintain seamless continuity of care, including coordination with physicians, nurse practitioners, physician assistants, pharmacists, and hospital case managers 

  • Maintain timely and accurate patient medical records utilizing electronic health record software 

  • Make informed decisions in a dynamic and fast-paced work environment 

  • Participate in community health initiatives such as blood pressure or immunization clinics, to promote Acadian Companies 

  • Perform other duties as assigned by the Acadian Health Operations Director or Medical Director 

 

QUALIFICATIONS 

  • Hold an Associate Degree in Nursing; Bachelors preferred 

  • Hold an active Registered Nursing license in the state of employment 

  • Current BLS and ACLS certifications established by the American Heart Association 

  • Current and valid state-issued driver's license determined by appropriate state requirements and acceptable driving record in accordance with Acadian's driving and insurability policies 

  • Effective verbal, written, and interpersonal communication skills 

  • Ability to provide effective coaching and leadership 

  • Ability to adhere to established stands for educational quality 

  • Familiarity with basic computer applications and functions

  • Team-oriented and able to work effectively and efficiently with others 

  • Ability to lift 50 lbs safely 

 

TRAINING REQUIREMENTS 

  • Orientation to job requirements

  • Completion of the Acadian Health clinical education program 

  • Completion of ride-along shifts with current Acadian Health Staff 

  • Clearance to perform; issued by an Acadian Health Operations Supervisor 

  • Participation in the Acadian Health Continuing Education program
