Wilson Health is looking for a direct hire Quality Measurement and Improvement RN for Wilson Health Medical Group in Sidney, Ohio (North Dayton, Ohio) area.

The Role You Will Contribute:
The Quality Measurement and Improvement RN plays a vital role in patient care beyond the hospital walls, using the latest technology.

Employment Status: FT
Working Hours: Monday- Friday 8:00am-4:30pm (40 hours a week)
Position Reports to: Chief Medical Officer

Essential Duties & Responsibilities:
Includes the following. Other duties may be assigned.
  • Primary point of contact with insurers regarding quality program requirements and adherence.
  • Ensures that documentation produced and reported complies with participation and reporting requirements.
  • Prepares and presents Quality summary reports to providers on an individual and group basis.
  • Participates in the development and implementation of long-term care coordination, including remote monitoring.
  • Provides data to help monitor provider and group performance related to quality programs and assists with strategies to improve performance.
  • Interfaces with and facilitates insurance company chart audits and results review related to quality and performance initiatives.
  • Collaborates with information technology department to ensure satisfactory support of quality program data capture and reporting.
  •  Collaborates with Wilson Health patient experience staff to ensure satisfactory support of WHMG data capture and reporting.
  •  Serves as liaison to the Wilson Health Quality Department for data sharing and reporting.
  •  Demonstrates a willingness and ability to communicate, cooperate and work collaboratively with co- workers.
  •  Demonstrates teamwork to go get all functions completed even when not specifically assigned.
  •  Demonstrates the ability to interact with patients during various processes such as, transition of care, med adherence, remote monitoring, etc.
  •  Demonstrates ability to use clinical knowledge to review documentation and educate patients.
  •  Demonstrates a willingness and ability to communicate, cooperate, respect and work with Managers, Physicians, Physician Assistants and APRNs. Shows dedication to help health care providers be efficient.
  •  Demonstrates the ability to complete task completely and accurately in an appropriate amount of time.
  •  Suggests ideas to explore innovative ways of accomplishing the goals of the program and the group in general.
  •  Protects patients' rights by maintaining confidentiality of personal information in accordance with HIPAA policies and procedures.
Qualifications:
  • Graduate of an accredited school of professional nursing
  • Current license as Registered Nurse in the state of Ohio
  •  BLS
  •  Minimum two years' experience in inpatient Med/Surg or two years' experience in outpatient care.
  • Experience in interaction with insurers representatives.
  •  Proficient with technology and able to instruct others on how to use different medical devices.
