- Certification Details
- Required certifications: BLS, ACLS, possibly others
- Job Requirements
- Must be AHA certified in BLS, ACLS
- Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals
- Vaccines or Positive IGG Titer needed. If Negative titer, booster vaccine required. No exemptions accepted for certain work areas
- Required annual Fit Test for all disciplines
- Hepatitis B vaccination series completion or declination to vaccine documentation
- Influenza Vaccine required starting Sept 4th, 2023, updated every year
- Completed physical exam or health statement prior to assignment
- 2 Step PPD OR IGRA within required timeframe, Chest X-Ray if positive TB record
- Proof of Varicella vaccines, immunity, or history verified by healthcare provider
- Valid Tdap within 10 years from the administered date
- Ascension Criminal Background Check Results
- Ascension EPLS
- Annual specialized training (Ascension Skills Checklist, Ascension Medical Surgical Exam, Ascension Core Competencies, NIH Certification)
- 12 Panel Drug Screen completed within 30 days and updated annually
- Ascension Badge Photo with specific background
- Verified education in accordance with applicable laws
- Ascension Confirmation of Assignment
- Score of 90% or higher on Ascension EKG Exam with only 2 attempts
- Ascension unit leader to upload Ascension Unit Specific Checklist within a week
- Pre-hire 6 hours attestation for Ascension Onboarding Requirements Education
- Ascension Nursing Contract Worker Onboarding done onsite
- Ascension Nova StatStrip training
- Ascension Influenza Vaccine Exemption/Declination during flu season
- Additional Details
- For Michigan hospitals, license verification through LARA not NURSYs but upload in NURSYs
- Review ALL license activity in all states via Nursys
- Exemptions not accepted for contact with pediatrics/infants, mon-immune pregnant women (L&D areas), non-immune immunosuppressed patients
