At the Department of Disabilities, Aging and Independent Living (DAIL), our Adult Services Division and the Choices for Care program helps older Vermonters and people with disabilities to access long-term care services & supports, in the setting of their choice.
As a Nurse Case Manager/Utilization Review Nurse I, you'll play a vital role in transforming lives. You will conduct clinical eligibility assessments and Utilization Reviews for Vermont's Long-Term Care Medicaid Choices for Care (CFC) Program, Brain Injury Program (BIP), Attendant Services Program (ASP) and for the Adult High Technology (AHT) Program. This position is based in Middlebury and will include regular travel to neighboring counties.*
This role offers the opportunity to collaborate with a supportive team while working under the guidance of experienced Nurse Administrators.
RN licensure is required, experience with coordination of care, knowledge of community resources and state resources preferred.
**Starting rate may be negotiable based on experience and qualifications.

Duties are performed in a facility, standard office setting and/or in the community. Extensive travel may be required, for which private means of transport must be available. Certain functions may require duty outside of normal work hours. Some resistance or objection to standards may be anticipated, requiring tact and diplomacy to achieve proper action. Duties involve need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers; and operational integration with various other health care reform initiatives. Critical assessment skills are necessary to promote personal and client safety.
Minimum QualificationsPossession of (or eligible for) licensure as a Registered Nurse (RN) in Vermont OR eligible to practice in the state of Vermont via a multi-state license AND three (3) years or more of professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.
NOTE: Must maintain Vermont or multi-state licensure as a Registered Nurse as a condition of employment.
Preferred QualificationsBachelor of Science in Nursing (BSN).
Special RequirementsReliable means of personal transportation is required for work out of the office or in the community and field based settings.
