This position is responsible for planning and coordinating with local area agencies to provide community-based services. Responsibilities Include:
Plans, organizes, and coordinates with local area agencies to provide education on RHA Health Services
Coordinates and creates relationships among hospitals, social service agencies, schools, primary care, law enforcement, other community stakeholders
Provide a link between the community and healthcare professionals.
Relies on direction and supervision to accomplish tasks.
Education, Licensure, and Experience required for the position include:
Requires a four-year degree in a human service field and three years of experience with the population served.
