This position is responsible for planning and coordinating with local area agencies to provide community-based services. Responsibilities Include:

• Plans, organizes, and coordinates with local area agencies to provide education on RHA Health Services

• Coordinates and creates relationships among hospitals, social service agencies, schools, primary care, law enforcement, other community stakeholders

• Provide a link between the community and healthcare professionals.

• Relies on direction and supervision to accomplish tasks.

Education, Licensure, and Experience required for the position include:

• Requires a four-year degree in a human service field and three years of experience with the population served.
