Position Summary
We are seeking a compassionate and highly organized Oncology Nurse Navigator to guide patients through the cancer care continuum. This role serves as a key point of contact for patients, families, and the multidisciplinary care team, ensuring timely access to care, coordination of services, and a seamless patient experience.
The Oncology Nurse Navigator provides clinical expertise, education, and support from diagnosis through treatment and survivorship, while helping to remove barriers to care.
Key Responsibilities
- Coordinate and ensure seamless, evidence-based care across the oncology continuum
- Conduct comprehensive assessments and collaborate with the multidisciplinary team to develop individualized care plans
- Provide patient and family education, support, and advocacy, addressing clinical and psychosocial needs
- Identify and address barriers to care to promote timely access and adherence to treatment
- Develop and deliver education materials and outreach programs for patients, staff, and the community
- Participate in quality improvement initiatives and maintain clinical expertise to support optimal patient outcomes
- Current Registered Nurse (RN) license in WI
- Bachelor of Science in Nursing preferred
- Minimum of 3–5 years of oncology nursing experience
- Oncology Certified Nurse (OCN) preferred or required within 24 months
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to manage multiple priorities in a fast-paced environment
Preferred Skills
- Experience in care coordination or navigation
- Knowledge of oncology treatment pathways
- Familiarity with EPIC
- Strong patient advocacy skills
