Job Summary
Coordinates communication and medical services between clinical staff, physicians, DMEs, and laboratories. Responsible for coordinating with other disciplines and agencies involved with patient's care.Bonus

Accountabilities
- Team Responsibilities: Promotes, maintains and documents communication between physicians and clinical staff regarding patient care and agency function. Confers with supervisors and attends team meetings. 20%
- Documentation Responsibilities: Documents orders received from physicians and lab results received from laboratories. 20%
- Professional Responsibilities: Promotes and expedites communication between home health clinical staff in the field and physicians and their medical staff regarding patient care and agency function. Coordinates patient lab schedules, lab orders, and lab results with clinical staff, laboratories, and physicians. Transmits patient clinical notes to DMEs as required for quality patient care. Maintains the highest level of professionalism and conduct at all times. Follows agency policies and procedures. 20%
- Clinical Responsibilities: Monitors results of telehealth monitors and reports results as appropriate to clinical staff and the patient's physician. 20%
- Administrative Responsibilities: Submits all paperwork/documentation according to agency policy. Participates in chart audits and other related Quality Improvement projects. 15%
- Accepts assignments outside normal duties when such fulfills an agency need. 5%
Associate's Degree
Graduate of an accredited school of Nursing program and approved by the South Carolina State Board of Nursing
Bachelor's degree in Nursing preferred.
One year experience as a registered nurse
Required Certifications/Registrations/LicensesLicensure to practice as a Registered Nurse in South Carolina
Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.
Required Knowledge, Skills, Abilities
Basic computer skills
Knowledge of office equipment (fax/copier)
Proficient computer skills (word processing, database)
Data entry
Other Required Skills/Experience
Driving is required for this position using the employee's personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies. Employee must pass driver training.
